CenterLight Health System SUPERVISOR OF CLINICAL ASSESSMENT in Flushing, New York
SCOPE OF PRACTICE:
Responsible for supervising, instructing, and guiding the Assessment Nurse Managers to ensure compliance with CenterLight’s policies, procedures, and all regulatory mandates, consistent with CenterLight’s mission, vision and value statements. Continuous review of charts and assessments to ensure that documentation meets quality standards and regulations. Serves as role model to staff with respect to ethical conduct, decision-making, and responsibility for quality and member satisfaction.
Supervises Assessment Nurse Managers to ensure that assessments are being completed in a comprehensive and thorough manner; assessments are entered properly into the UAS and Care Compass and the information is accurate and available for care planning purposes; Assessment RNs report back on any unusual or complex member issues so that appropriate and timely actions can be addressed.
Participate in the orientation and training of Assessment Nurse Managers to ensure comprehensive knowledge of CenterLight’s health risk assessments, analytical and technology tools and systems, member population understanding, and compliance-related policies and procedures, among other areas.
Provide continual direction and support to Assessment Nurse Managers to guide and support effective clinical skills, strong critical thinking, and appropriate assessments and care planning.
Serve as role model for staff so they can develop their own skills and competencies.
Monitor assignments and productivity of the Assessment RNs.
Participate in Performance Improvement Activities.
Work in collaboration with Directors, other supervisors, and CenterLight staff.

Weekly Hours: 40
Schedule: 8:30 AM - 5:30 PM
Experienced RN/Clinical professional in the home health care field.
Previous experience with supervision and educating of nurses/staff.
10-15 years of experience in health care; geriatrics, home care, care management, acute care and/or long term care nursing field, with experience educating and supervising.
Knowledge of requirements of a Medicare-Medicaid integrated health plan covering the dual elderly population.
Knowledge of community-based long term care services to dual eligible and frail adult population.
Knowledge of physical health, aging and loss, and frequently used medications and side effects, Alzheimer’s disease and dementia, behavioral health, and community resources or supports.
Comprehensive understanding of federal and state regulatory requirements with respect to health care coverage and services of various public programs.
Excellent assessment skills, interpersonal skills along with demonstrated ability to handle situations requiring immediate and urgent attention, while addressing routine and long term objectives.
