Sign-On Bonus $10,000 with 2-year full-time work commitment

Position Summary:

Performs physical therapy treatments for patients referred to department. Conducts patient evaluation, performs documentation of patient progress, treatment planning and completes discharge functions.

Qualifications:

Required:

• Possess minimum of 5 years formal education and Bachelor/Master Degree/Doctorate in accredited Physical Therapy program.

• Licensure in State ofMichiganrequired.

• Trained and educated in age-specific categories being served: infant, pediatric, adolescent, adult and geriatric.

• Capable of spending 10-25%25%20of time lifting, carrying, moving items weighing 25 pounds or more.

• Ability to effectively communicate and present self to people in courteous, tactful and poised manner.

Preferred:

• Current membership through the American Physical Therapy Association
