General Description

This is a full-time Program Supervisor/Medical Case Manager (MCM) position for the HIV/AIDS Ryan White Part B Program (RWPB). The Program Supervisor/MCM will develop and sustain relationships with organizations and health and community centers within Coconino, Apache, and Navajo counties, including the Navajo, Apacheand Hopi reservations in Arizona.Coordinating care involves linking clients to dental, primary care, HIV specialty care, behavioral health, and support services. In addition, the Medical Case Manager will help clients navigate different insurance systems and access other non-Ryan White resources and services available to HIV patients.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Manage the RWPB grantdeliverablesfor Northern Arizona,includingproviding outpatient medical, medical, and non-medical case management for persons living with HIV/AIDS in the service area.
  • Fulfill Medical Case Manager role to coordinate the care of clients.
  • Ensure RWPB contract requirements are met, and Standards of Care are followed.
  • Enhance and develop relationships with North Country clinics, county health departments, the Arizona Department of Health Services, Indian Health Care Centers on the Navajo, Apacheand Hopi reservations, and other agencies serving persons living with HIV in the service area.
  • Carries out supervisory responsibilities in accordance with the organizations policies and applicable laws.
  • Utilize various platforms for program monitoring, data collection, and analysis. Maintain program data for reporting and evaluation.
  • Develop applicable policies and procedures and implement process improvement efforts to ensure clients receive the services they are eligible for.
  • Facilitate, organize, and utilize systems for tracking client services, demographic information, and program budgets.
  • Maintain accurate and complete client files, treatment plans, and program databases, including entering client information into EHR and CAREWare.
  • Create and develop a support group for clients enrolled in the RWPB program.
  • Effectively demonstrate cultural competence in working with diverse populations.
  • Promote North Countrys HIV services to organizations and groups.
  • Maintain professional working relationships and collaborative partnerships with all clinic staff, clients, healthcare institutions, academic institutions, community organizations, and other stakeholders.
  • Monitor program/project spending and adhere to program budgets.
  • Participate in program sustainability building (i.e., grant writing, fundraising), as applicable.
  • Interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints; and resolving problems.
  • Travel throughout service areas and regions for meetings, visiting clients, and training as necessary.

Education

4-year college degree or equivalent experience resulting in knowledge of a field related to the job, including but not limited to public health, social work, and infectious disease.

Experience

Minimum Qualifications/Skills Experience in Public Health or Social Science field. Must have excellent organizational skills, as well as verbal and written communication skills. Supervisory and budget experience strongly encouraged. Strong data analysis and computer skills, Excel and Word required. Must be able to work independently, and manage multiple priorities. Competent working with people of other ethnic backgrounds, low incomes, and in rural areas . Must be able to maintain strict client confidentiality. Some travel required. Able to show flexibility when policies change or with client requests.
