Job Summary

Provide leadership and day-to-day management of the case management efforts within Sanford. Leads case management initiatives in different settings and environments. Enhance the throughput of patients and reimbursement received through appropriate status utilization, coordination of care, length of stay management, and discharge disposition management to ensure the delivery of quality healthcare and development of services according to the mission, vision, and strategic goals of Sanford. Monitors patient progress and identifies issues and concerns regarding progress. Understand and provide insight into evaluating current process improvement strategies including quality, methods, and ability to maintain focus on the continuous improvement of processes, products and services. Coordinates and manages care for patients, including those with complex needs. Applies key aspects of continuous improvement processes and evaluates impact on quality. Manage processes to support attainment of goals within department and organization. Knowledgeable of industry standards, governing bodies, and regulations. In addition to assisting with regulatory and/or enterprise reporting, ability to report on and analyze results for process improvement strategies.

Work collaboratively with the healthcare team to coordinate, facilitate, and communicate discharge planning needs and coordination of care. Adjusts to new or changing assignments, processes, and people. Being a positive role model for staff to coach, educate and support both the employees and organizational growth. Works with inter-professional teams to create an overall patient care plan. Mentors others in resolving issues and concerns regarding case management processes. Determines individual and team competency requirements, vulnerabilities, and learning needs. Assumes management responsibilities such as payroll, scheduling, day-to-day staffing and crucial conversations in collaboration with human resources and leadership. Identifies opportunity for personal and professional growth and pursues educational opportunities.

Qualifications

Bachelor's degree in nursing, social work or other healthcare related field required. Graduate from a nationally accredited program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA), or Council on Social Work Education (CSWE).

Minimum of three years clinical practice experience. At least two years of case manager/care coordination experience. Must have at least two years demonstrated leadership experience.

Position requires a current unencumbered license with the state where the practice of nursing (RN) or social work (SW) is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Incumbent must obtain and subsequently maintain required department specific competencies and certifications including Basic Life Support (BLS) within 12 months of hire. Within two years from date of hire into position, desirable to obtain one of the following: Case Management Certification (CCM), Care Coordination and Transition Management (CCCTM), Certified Nurse Manager and Leader (CNML), or additional professional certification in nursing specialty in field of study.

A social work license must be in good standing in state(s) of practice:

In Iowa, must have one of the following:

Licensed Bachelor Social Worker (LBSW)

Licensed Master Social Worker (LMSW)

Licensed Independent Social Worker (LISW)

In Minnesota, must have one of the following:

Licensed Social Worker (LSW)

Licensed Graduated Social Worker (LGSW)

Licensed Independent Social Worker (LISW)

Licensed Independent Clinical Social Worker (LICSW)

In North Dakota, must have one of the following:

Licensed Baccalaureate Social Worker (LBSW)

Licensed Master Social Worker (LMSW)

Licensed Clinical Social Worker (LCSW)

In South Dakota, must have one of the following:

Social Work Associate (SWA)

Social Worker license (SW)

Certified Social Worker (CSW)

Certified Social Worker - Private Independent Practice (CSW-PIP)
