Hillside Family of Agencies Mental Health Clinician I School Based/Social Worker(LCSW, LMSW, LMFT, LMHC, LCAT) in Fairport, New York

Job Summary

The Clinician I will assess and deliver quality mental health care to youth and family in Community, Home, School, or Residential setting under appropriate supervision.

This master's level, clinical position is school-based, and provides individual, family and group therapy, as well as case management services. The program serves youth with intellectual and developmental disabilties with a co-existing mental health diagnosis. Clinicians work in a BOCES School program and also offer services in the home/community. You must be willing to transport youth in your vehicle, if needed, and carry the required amount of automobile insurance. You must also be a licensed therapist (LCSW, LMSW, LMHC, LCAT, LMFT) or have the ability to immediately acquire a limited permit. Experience with the population is preferred.

This is a Monday - Friday full-time (40 hours) position on an 8:00am-4:00pm schedule.

Essential Job Functions

  • Assist the Clinical team in the assessment of Hillside youth and family needs through clinical interventions to treat and achieve positive outcomes.

  • Create, implement, evaluate, and modify, as needed, individual treatment plans to meet assessed client’s unique needs as a member of a multi-disciplinary team.

  • Assess and facilitate referrals to Hillside services and resources provided by the treatment team.

  • Provide individual and group therapy, as well as family therapy, based on client needs.

  • Collaborate with other informal supports, community, and mental health providers to provide needed services to clients.

  • Develop, maintain, and retain all required documentation in a compliant and timely manner including, but not limited to, clinical assessments, treatment plans, and progress notes.

  • Act as the primary contact for the treatment plan for youth being treated.

  • May provide transportation for youth as needed to appointments, meetings, or appearances.

  • Ensure Hillside policies, procedures and all applicable governmental laws/regulations/guidelines are followed.

  • Youth ACT and Therapeutic Foster Care programs: On call responsibilities as scheduled.

  • While this job description covers many aspects of the role, employees may be required to perform other duties as assigned.

Education & Experience

  • Master’s degree required

  • Must be license eligible

Special Requirements

  • Licensed Master Social Worker, Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, Licensed Psychologist, Licensed Mental Health Counselor, or Licensed Creative Arts Therapist

  • If not licensed, obtain a limited permit upon hire and successfully pass licensing exam by the end of permit period.

  • Unrestricted, valid NYS driver’s license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.

  • Ability and willingness to travel

  • Must be able to work a variable scheduling including evenings and weekends

  • The following physical demands are required:

  • Occasional sitting (up to 4 hours)

  • Occasional standing (up to 1 hour)

  • Occasional walking (up to 2.5 hours)

  • Occasional driving (up to 6 hours)

  • Continuous balancing (up to 8 hours)

  • Occasional balancing, bending, stooping, climbing, kneeling, pushing, pulling, reaching forward or down, reaching overhead, running, and twisting (up to 2.5 hours)

  • Weekly lifting up to 50 lbs. (community-based staff may be required to lift an empty wheelchair weighing up to 50 lbs.), 10-15 lbs. on a daily basis

  • Weekly carrying up to 55 lbs. approximately 10-100 ft., 10-15 lbs. on a daily basis

  • Manual dexterity is required, including the frequent ability to grasp in both hands and continuous use of fine manipulation skills in both hands (approximately 1-2.5 hrs.)

  • Occasional exposure to dust, fumes, gases or chemicals is apparent.

  • Protective clothing or equipment as required: Close toed shoes; Protective Shields, Universal Precaution PPE, Gloves for toileting and food preparation

  • Ability to change positions as needed

$58,500 Minimum pay rate, $75,570 Maximum pay rate, based on experience.
