As a Certified Physical Therapy Assistant (PTA), you are a valuable member of the interdisciplinary team that focuses on providing compassionate, quality care and producing positive outcomes for your patient’s recovery. We provide home-based therapy for patients as directed by the attending physician and by following the goals created by the Physical Therapist. You have the unique opportunity to work with the patient in their home and be part of their progress and recovery to the highest level of independence possible.

WORK SETTING:

You'll provide comprehensive skilled services as ordered by the attending physician. You will educate patients and their family members on how they can complete exercises to develop strength, balance, endurance as well as confidence. All the while ensuring the safety of the patient. You are accountable for completing accurate documentation the same day as you provide the service. This ensures the most accurate documentation. We are a Medicare/Medicaid certified home health agency and meet all Conditions of Participation (CoP’s) per Medicare requirements.

PRIMARY RESPONSIBILITIES:

  • Plan of Care: Provide skilled physical therapy as ordered by the attending physician and observe and monitor patient conditions and notify the Physical Therapist of any changes in the patient’s condition.

  • Minimize Patient Risk: Assesses functional problems and provides intervention and, in conjunction with the Physical Therapist works toward goals to improve patient functional status

  • Family Educator/Advocate: Counsel patient and family on the disease/injury and how to manage in all phases of physical therapy. Instruct patient and family members on proper use of equipment.

  • Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.

  • Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.

    REQUIREMENTS:

  • Physical Therapy Assistant (PTA) with current license in Iowa

  • Minimum one-year experience as a PTA in Home Care or an acute care setting. Home Care experience highly preferred.

  • Valid driver's license and auto insurance in your name as a driver

  • Capable of all physical demands
