Is responsible for the evaluation, clinical assessment, treatment, plan, performance of Speech-Language interventions, and treatment plan progression for physician-referred patients that demonstrate speech, language, hearing, and oral myofuctional disorders. Responsibilities also include consulting with physicians and the health care team regarding patient care, participating in team and patient care, participating in team and patient/family conferences, educating physicians and associated to speech-language interventions to insure continuity of care, and completing all required paper work such as clinical records, care plans, and charge sheets.

MINIMUM QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1.Master's Degree from a school of Speech Language pathology. 2. Current West Virginia SLP Therapy Licensure, or applicable state licensure where services will be performed. 3. CPR certification within 30 days of hire date. 4. Valid driver's license.

PREFERRED QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1.Member of (ASLHA) American Speech Language Hearing Association. 2.Must possess valid WV driver's license and must have automobiles insurance of $1000,0000/3000,0000 on his/her vehicle.

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. They are not intended to be constructed as an exhaustive list of all responsibilities and duties. Other duties may be assigned.

  1. Performs evaluation procedures according to policy/procedures according to policy/procedure or standard of care including but not limited to: dysphagia evaluation, voice evaluation, communication evaluation, cognitive assessment, language evaluation, hearing screens.
  2. Safely and effectively delivers and analyzes speech-language interventions as per clinical policies and procedures.
  3. Implements home programs, meets patient/family educational needs as per policy and procedure.
  4. Promptly and thoroughly completes required clinical documentation including evaluations, progress reports, discharge summaries, and care plans.
  5. Instructs and delegates restorative speech language programs to the nursing staff to insure continuum of care as per policy and procedure.
  6. Calls patients timely and follows visit pattern.
  7. Performs CPR when directed by patient's/appropriate representative's wishes.
  8. Demonstrates bag technique per policy and procedure.
  9. Apply and maintain equipment according to the Policy and Procedure Manual.
  10. Follows supply requisition systems as per policy and procedure.
  11. Evaluates the effectiveness of the plan on a regular basis.
  12. Coordinates the care team in order to facilitate a change in the level of care in compliance with the change in level of care standard.
  13. Identifies the stages of death and dying
