Job Overview:

The Sr. Program Assistant provides direct social service support to patients as well as assesses and plans complex discharge planning needs for the Adult Psychiatric Inpatient Unit.

The psychiatric inpatient 22-bed, Dual Diagnosis Mental Illness unit, specializes in treatment of patients with autism and intellectual disabilities and varying medical complexities.

Qualifications:

Required:

• Bachelor's degree in the Behavioral Health field of study

Scheduling Requirements:

• Shift working, 8:00 4:30pm each day

• FT

Essential Functions:

• Interface with social work team to collaborate on social supports, housing and treatment needs upon discharge

• Participate in interdisciplinary treatment team planning rounds to assess discharge planning needs, identify obstacles and communicate on-going interventions

• Educate and engage patients and families with availability and eligibility criteria for outpatient programs/services benefits to meet their unique needs

• Creatively negotiate, coordinate, and advocate for patients with community based service to assure the highest level of needed supports, continuation of care and wrap around services

• Collaborate with the team to identify, send, and follow-up on all appropriate referrals

• Build and maintain relationships with outpatient referral agencies and programs throughout Union County that will best serve our patient population

• Document all discharge planning related notes and referrals in the Medical Record

• Other job duties as assigned
