Hospital Sisters Health Systems (HSHS) Social Worker-MSW/LCSW in Effingham, Illinois

  • HSHS Clinic Setting

We're seeking a Social Worker-MSW/LCSW in an HSHS clinic setting to provide therapeutic services to patients dealing with a wide range of issues by counseling them on managing ongoing health conditions and making positive behavior changes.

Responsibilities include:

  • Meeting with patients to assess their histories, condition, and needs.

  • Interviewing family members when needed.

  • Providing and communicating recommendations.

  • Collaborating with provider(s) to produce a treatment plan that best meets the needs of the patient.

  • Monitoring patients’ progress.

Social Worker-MSW play an essential role in many of the aspects of patient care, including helping patients and their families navigate the medical system, assessing and monitoring patients' and family members' mental and emotional health, providing short term counseling and therapy, and communicating patient needs and concerns to the larger medical team.

Qualifications

Education

Master’s Degree in Social Work is required.

Experience

3 years of discharge planning is preferred.

3 years in social service field is preferred.

Certifications, Licenses and Registrations

Wisconsin: Licensed as a Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW), or Licensed Clinical Social Worker (LCSW) is required.

Illinois: Licensed as a Licensed Clinical Social Worker (LCSW) is required.

Basic Life Support (BLS) is required.
