M Health Fairview's Advance Care Planning department has an immediate opening for a Documentation Specialist.
This is a 1.0 FTE (80 hours per two week pay period), benefits-eligible opening. This is a Monday - Friday day shift opening.

Responsibilities Job Description
The Document Specialist works both independently and collaboratively to support the Advance Care Planning (ACP) and Surrogate Decision-Maker needs of M Health Fairview and Ebenezer in all regions. The Document Specialist validates and scans legal documents identifying Surrogate Decision Makers, and updates the medical record in alignment with department and system protocols. This position also assists the department to provide staff, patient, and community support related to Advance Care Planning and Surrogate Decision-Making.
Perform validation of legal documents in alignment with department protocols and state statutory requirements
Scan legal documents in compliance with department and system protocols
Update patient and surrogate-decision maker demographics and authority in compliance with department protocols and state statutes.
Assist providers, staff, patients, and other customers with Advance Care Planning and Surrogate Decision Maker requests and inquiries in alignment with department protocols.
Collaborate to measure, evaluate and optimize outcomes to meet department and system strategic goals
Qualifications
Required
Education
Associates degree or equivalent combination of education/experience
Experience
Two years demonstrated knowledge/experience in:
Working both independently and within a collaborative team while achieving goals
Utilizing organizational and communication skills
Building relationships with system teams to achieve specified outcomes
One year demonstrated knowledge/experience with health-care related legal documents or related experience
Preferred
Experience
Knowledge of the purpose and process of Advance Care Planning
Knowledge of ACP process and resources within health care systems
Proficiency in Microsoft applications
Proficiency in Epic application

Ability to prioritize and work with a fluctuation in workload while working independently
Ability to adapt to change and engage in ongoing process improvement
