Position: Licensed Practical Nurse (LPN)

The Grand Rehabilitation and Nursing Center at Pawling is seeking dedicated LPNs to join our team. With flexible full-time, part-time, and per diem shifts available, you can find a schedule that works for you.

Key Responsibilities:

• Collaborate with physicians and care associates to implement and coordinate resident care.

• Conduct regular health and wellness checks for residents.

• Assist in developing and updating individualized service plans for optimal care.

• Communicate effectively with residents, families, and healthcare providers.

• Utilize the nursing process (assessment, planning, implementation, evaluation) to deliver outstanding care.

  Qualifications:

• Current and valid NYS LPN License in good standing.

• Strong teamwork skills and the ability to thrive in a collaborative environment.

• Previous experience in a nursing home or hospital setting is preferred but not required.
