Position Summary

Under the direct supervision of the Chief Medical Officer, the Licensed Clinical Social Worker (LCSW) is responsible for providing behavioral health services at the Zufall Health Center sites. The LCSW provides direct se rvice s to pati e nt s, families and groups, provides individual and family counseling, and engages in crisis intervention. Identifies opportunities for developing integrated Behavioral Health Services in the primary care setting and serves as a resource to the provider staff.

Essential Functions, Duties and Responsibilities

  • Review referrals and assess potential clients that are referred by the provider staff, Care Team and other patient advocates. Make initial contact with patient to determine appropriate services needed.

  • Complete a Comprehensive A ssess ment and Treatment Plan ba se d on patient' s hi s tory and relative to the pr esent ing problem. Follow established protocols and methodologies

  • Collaborate with patients in the development of treatment plans and measurable goals and outcomes.

  • Review cases periodically with supervisor and/or consultant mental health professionals .

  • Maintain patient records well documented and in a timely fashion.

  • Participate in crisis intervention as re s pon se for patient s dan gero us to se lf/other s/ property.

  • Lead group sessions and activities, and develop curricula for specific initiative (for example, Healthy Lifestyles). Conduct pre- and post-intervention assessments and patient surveys to evaluate effectiveness of programs.

  • Participate and present in care conferences, patient groups, educational programs for staff and for patients, staff meetings, staff trainings and other venues as assigned .

  • Represent Zufall Health Center at community service organization meetings, behavioral health agencies and collaborating entities .

  • Participate in outreach and community educational activities as part of the Care Team as assigned by the Chief Medical Officer . Participate in in-service educational programs and continuing education. Teach and train other health center staff and students as needed.

  • Follow established departmental policie s and procedures regarding quality assurance, sa fety , environmental and infection contro l sta ndards. Assist in the development of relevant policies and procedures as the department and services evolve.

  • Participate in medical/clinical staff meetings and genera l staff meetin gs.

  • Ensu r e patient co nfidentiality under HIPAA regulation s.

  • Perform other dutie s as assigned.

Knowledge, Skills and Abilities

  • Excellent customer focused interpersonal skills to interact in an effective manner with patients, families, the interdisciplinary health care team and community agencies with diverse opinions, values, and religious and cultural ideals.

  • Excellent verbal and written communication skills

  • Culturally sensitive and knowledgeable of multicultural mores, beliefs, and customs

  • Bilingual in Spanish/English

  • Excellent computer skills, experience with electronic medical records a plus

  • Ability to follow instructions, given either orally or in writing.

  • Ability to work independently or with little supervision.

  • Excellent organizational skills such as problem solving, decision making, priority setting, and work delegation.

  • Ability to complete assignments in a timely manner.

  • Ability to work as a team member.

  • Ability to maintain HIPAA compliance, corporate compliance and client confidentiality.

    Education, Training and Experience

  • Ma ste r' s D eg ree in Social Work

  • Licensed in New Jersey as an LCSW

  • Minimum of three years post graduate experience in clinical/ambulatory care setting

  • Experience in working with groups.
