Job Description Summary

The Order Tracking LPN will ensure all diagnostic and surgery orders meet medical necessity under the Southeast Health protocol. Actively participates in on-going efforts to identify and educate providers and patients regarding evidence-based and cost effective alternatives for excellent care. Collaborates with Care Management Staff members, several local medical offices and Financial Assistance Centers to review services requested, and provide feedback to the ordering provider if desired services do not meet medical necessity.

Job Description

Essential Functions

  • Responsible for organizing and maintaining all incoming orders for outpatient diagnostic procedures and surgeries.

  • Responsible for obtaining history and physicals, office notes, and required documents prior to scheduled surgeries.

  • Review of clinical chart for compliance reimbursement and compliance with medical necessity guidelines, to include facility and physician records.

  • Responsible for entering physician’s orders into electronic medical record and moving to appropriate folders.

  • Responsible for calling physician’s offices and obtaining orders when necessary.

  • Act as clinical liaison between facility and physician offices coordinating/scheduling patient’s treatment plan.

  • Conducts telephone conversations with patient, physician, physician office personnel to discuss clinical information and issues, to include financial requirements, status and schedules.

  • Responsible for identifying medically urgent and emergent patients for outpatient and inpatient services underinsured and private pay patients.

  • Coordinates physician to physician patient status calls when deemed necessary.

  • Refers patient accounts to outsource vendors for assistance with financial needs.

  • Identify patient health risk factors, such as allergies, pre-existing conditions, or prior treatment, to anticipate obstacles.

  • Schedules patients for diagnostic tests from orders.

  • Actively supports the mission, vision and values of the hospital and department.

    Qualifications

Minimum Education Required

  • Graduate of an accredited school of nursing.

  • Current LPN Licensure in the State of Alabama.

Minimum Experience Required

  • Five (5) years of LPN experience

Required Skills/Abilities

  • Must be able to read, write and speak English.

  • Written, verbal and interpersonal skills required.

  • Basic Computer skills and comfort with Windows operating system required.

  • Basic typing skills required.

  • Annual completion of Department Specific Competency requirements as delineated in unit specific competency tool.
