Job Summary:

Acute Home Healthcare, Inc. are seeking an experienced and compassionate Speech Therapist (Speech-Language Pathologist) to join our home healthcare team, serving patients throughout Metro Detroit and surrounding areas. This role offers flexible hours and immediate opportunities for employment. As a Speech Therapist in home healthcare, you will provide therapeutic interventions to help patients improve communication, cognitive, and swallowing functions, empowering them to regain independence and enhance their quality of life.

Key Responsibilities:
  • Patient Assessment: Conduct comprehensive evaluations of patients' speech, language, cognitive, and swallowing abilities to determine therapeutic needs.
  • Care Planning: Develop and implement individualized treatment plans tailored to the specific needs of each patient, focusing on improving communication skills, cognitive functioning, and safe swallowing.
  • Therapeutic Interventions: Provide direct therapy to address speech, language, voice, fluency, and swallowing disorders, using evidence-based techniques and tools.
  • Patient and Family Education: Educate patients, caregivers, and family members on effective communication strategies, home exercises, and dietary recommendations to support safe swallowing and communication.
  • Progress Monitoring: Track and assess patients' progress regularly, adjusting treatment plans as necessary to meet established goals and improve functional outcomes.
  • Collaboration: Work closely with other healthcare professionals, such as occupational therapists, physical therapists, nurses, and physicians, to ensure comprehensive and integrated care.
  • Documentation: Maintain accurate and detailed clinical records, including evaluation results, treatment plans, progress notes, and discharge summaries.
  • Patient Advocacy: Provide compassionate support to patients and families, promoting independence and improving patients' quality of life.
Qualifications:
  • License: Active, unrestricted Speech-Language Pathologist (SLP) license in the state of Michigan.
  • Education: Master's degree in Speech-Language Pathology from an accredited institution.
  • Certification: Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA preferred. Current CPR certification required.
  • Experience: Previous experience in home healthcare or rehabilitation settings is preferred, but new graduates with clinical field experience are welcome to apply.
  • Skills: Strong communication, critical thinking, and organizational skills, with the ability to work independently in patients' homes.
  • Transportation: Reliable transportation required. Must have a valid driver's license and auto insurance, as travel to patients' homes is necessary.
  • Background Check: Successful completion of a background check and drug screening required.
