Delta County Case Manager in Delta, Colorado
POSITION: Case Manager
Provide case management services to clients approved for Colorado Works and Child Care Assistance Program (CCCAP) benefits according to State and County regulations. Communicate the purpose and expectations of the Colorado Works Program, aka Temporary Assistance for Needy Families (TANF), to individuals approved for assistance. Conduct assessments on Colorado Works clients to identify job readiness, strengths and barriers and develop an Individualized Plan (IP) to work toward self-sufficiency. Determine initial and continuing eligibility for the CCCAP. Determine eligibility for the county s General Assistance Program.
DUTIES: * Facilitate individual and family assessments to help identify job readiness, individual strengths, and barriers to self-sufficiency; assess clients regularly and revise the IP to achieve the goal of self-sufficiency. Explore options and resources available to assist the client meet family needs. * Ensures all Colorado Works recipients are in a State or County defined work activity while receiving cash benefits; monitor work activity attendance/participation and progress; timely and accurately document the activities in the Colorado Benefit Management System (CBMS). * Assist Colorado Works clients in developing an Individualized Plan (IP) that details participant goals, program expectations, client/county responsibilities, and county supportive services; utilize case management techniques to ensure IP engagement, re-assessing, and re-evaluating client needs to move the client toward a self-sufficiency status within a reasonable amount of time; enforce sanctions against the client for willful non-compliance

REQUIREMENTS: * A high school diploma or GED equivalent is required. * Two (2) years of experience in case management or related experience is required. * Candidate must pass a background investigation. * Must possess and maintain a valid Colorado Driver's License with a satisfactory driving record.
- Combination of education and experience could waive specific requirement.
PREFERRED SKILLS: * Bachelor s degree from an accredited college or university in a human services field, career counseling, or a related field is preferred. * Experience working with low-income, unemployed, under-employed and/or diverse populations is preferred. * Bilingual in Spanish and English languages is preferred but not required.
WAGE: * $22.10 to $24.31 per hour without a related bachelor's degree * $24.54 to $26.99 per hour with a related bachelor's degree

SHIFTS: Full-time - 40 hours per week
