The Clinical Pharmacist is a dynamic and experienced Pharmacist knowledgeable in evidence-based pharmacy practice. This includes knowledge and implementation of safe, effective and cost-effective selection and utilization of medications for our patients. Serves as point person between our Centers providing education to physicians, advanced practice providers, nurses, pharmacy staff and other stakeholders as needed. Manages personnel, including coordinating employees' work and providing guidance and feedback to Medication COE.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES:

  • Review and verify prescriptions and medication order for appropriateness, including safety and efficacy.

  • Perform prospective drug utilization reviews, assessing for proper dosage, use directions, drug interactions, duplicate therapies, and any other regimen-related considerations and make interventions as needed.

  • Assist the department in addressing physician consults as needed. Provide timely retrieval of reliable drug information from available sources.

  • Manage pharmacotherapeutics services in accordance with institutional practice standards and or protocols.

  • Develop programs/guidelines that improve practice and patient outcomes.

  • Provide efficient cost-effective patient centered care.

  • Provide verbal and written consultations concerning medication therapy on a daily basis.

  • Participate in implementation and maintenance of guidelines and order sets.

  • Participate in the educational experience of pharmacy residents and pharmacy students.

  • Provide presentations, publications and other informative activities related to specialty practice.

  • Represent the pharmacy department on applicable committees and teams.

  • Provide input and guidance for management of medication formulary.

  • Provide operational support to the pharmacy, including dispensing and compounding.

  • Follow established department policies, procedures, objectives, quality assurance programs, and safety, environmental and infection control products.

  • Supporting and facilitating the QI process through ongoing development and monitoring of performance improvement indicators.

  • Growth and facilitation of Pharmacy work teams to meet quality, clinical standards of care.

  • Assisting the Director to provide effective and efficient use of all resources and for ensuring budget and value proposition targets are met.

  • Performs other duties as assigned and modified at manager’s discretion.

KNOWLEDGE, SKILLS AND ABILITIES:

  • Knowledge of and experience with state and federal insurance programs

  • Familiarity with appropriate local, state, and federal pharmacy-related regulations

  • Ability to work effectively, both independently and as part of a team

  • Ability to supervise and communicate effectively with staff, management and executives

  • Strong organization and time management skills and multi-tasking abilities

  • Must be detail oriented

  • Ability and willingness to travel locally, regionally and/or nationally up to 10% of the time; flexible to work evening, weekends and/or holidays as needed

  • Spoken and written fluency in English

EDUCATION AND EXPERIENCE CRITERIA:

  • Must have a License that is in good standing with State Board of Pharmacy

  • Doctorate of Pharmacy is required (PharmD); Pharmacy residency training is preferred, but not required

  • Experience with supervising Pharmacy Technicians

  • Experience in a managed care setting (HMO, MSO, PBM) is a plus

  • Certified to provide immunization is a plus

  • Completion of Medication Therapy Management certificate program or Ambulatory Care Certificate

  • Basic Life Support and Advanced Cardiovascular Life Support certification required

  • Experience teaching is a plus

  • Work Authorization/Security Clearance
