MORE ABOUT THE POSITION:

The Clinical Manager is a qualified professional responsible for the assessment, coordination, supervision and evaluation of the designated clinical activities.

Qualifications: (Preferred unless required by law)

Must be currently licensed as a Registered Nurse in the state the services are provided and must attend continuing education as required

· Minimum of two years of clinical experience in which one has been in a supervisory or administrative role in a home health care setting

· Maintain current BLS/CPR certification from either the American Heart Association (AHA) or the American Red Cross (ARC)

· This position requires that the employee is able to read, write, speak and understand the spoken English language to ensure the safety and wellbeing of our patients, customers, and clients when responding to their medical and physical needs.

RESPONSIBILITIES/ACCOUNTABILITIES:

  1. Provides superior customer service to internal and external customers and patients.

  2. Accountable for the care and outcomes that the staff provides.

  3. Meets with clinicians to review patient caseloads, and assess the quality of care, coordination of disciplines involved, and appropriate allocation of services.

  4. Mentors new clinicians and current staff.

  5. Performs joint visits with clinicians when needed to assess and document clinician ability to manage patient needs using current assessment tools, leading to excellent patient outcomes.

  6. Analyzes patient data per clinician, noting trends and working with clinicians to optimize patient outcomes.

  7. Ensures fair and consistent treatment of all clinicians relative to caseloads, productivity, and geography.

  8. Collaborates with clinicians to assist with managing scheduling and performance improvement regarding patients.

  9. Facilitates weekly multi-disciplinary team meetings.

  10. Operationalizes agency goals within the scope of responsibility of the role.

  11. Collaborates with other members of the care team when needed.

  12. Plans and manages time effectively and productively.

  13. Participates in task forces designed to improve patient outcomes.

  14. Ensures that daily operations incorporate administrative and clinical performance improvement.

  15. Actively participates in the quality assurance program.

  16. Complies with and promotes adherence to applicable legal requirements, standards, policies and procedures including those within the Compliance and Ethics Program, Standard/Code of Conduct, Federal False Claims Act and HIPAA.

  17. Identifies and resolves issues impacting clinical services.

  18. Assess staffing needs for their respective team.

  19. Acts as a resource to clinicians.

  20. Provides direct patient care as needed.

  21. Adheres to fiscal responsibility for clinical department.

  22. Allocates resources appropriately.

  23. Serves as an active participant within the clinical leadership team.

  24. Raises clinical issues and awareness to the Administrator.

  25. Performs other duties as assigned
