Summary:
Under the guidance of the Clinical Nurse Manager, provides nursing services to assist patients in achieving optimal physical, mental and social well-being. Monitors patient outcomes. Communicate with the physician who is responsible for the home health plan of care and other health care practitioners as appropriate related to the current home health plan of care. New Grad RN Case Manager on interim basis until fully independent within 6 months. .
Responsibilities:
Assessment of physical, emotional, socio-economic needs and environmental needs of patients and their families.
Provides direct skilled nursing care in accordance with the physician orders and documents care timely. Any skills not signed off by the preceptor/manager, will be the responsibilities of the new graduate and clinical nurse manager to prove competency prior to delivery of that specific care.
Observes, evaluates and reports to the physician the patient’s physical and mental condition; changes in condition; and termination of care when goals are attained, or when termination of care is required according to agency policy.
Provide patient, family and caregiver counseling and education.
Participates in the agency quality assurance and performance improvement plan, and agency-sponsored inservice training.
Under the direction of the preceptor and/or clinical manager, develops implements and evaluates the plan of care in partnership with the patient, representative (if any), caregiver and physician.
After the first 90 days of employment, independently supervises and provides clinical direction to the LPN. Fulfills these responsibilities under the direction of the preceptor or clinical nurse manager within the first 90 days.
Identification of patient and family needs for other home health services and initiates referrals when appropriate under the direction of the clinical nurse manager.
Fulfills all compliance responsibilities related to the position.
Performs other duties as assigned.
Other Information:
Required: Requires strong research skills, clear and concise interpersonal communication skills. Knowledge of systems integration.
Minimum Experience: one year
Desired: Bachelor's Degree in Nursing; Experienced in qualified home health agency.
Working Conditions:
Manual: significant manual skills/motor coord & finger dexterity
Occupational: Some occupational risk
Physical Effort: Very Heavy effort. May exert up to 50 lbs. force
Physical Environment: Some exposure to dirt, odors, noise, human waste, etc.
