As a Patient Access Liaison, you’ll contribute to our success in the following ways:
Makes live calls in partnership with Intake departments to assess non-admit status patients to follow for care with urgency.
Reviews documentation, including orders, History and Physicals (H&P’s)and other documentation to assist in the determination of eligibility.
Works with referral partners to obtain necessary clinical information to assist in establishing hospice eligibility
Ensures contact with patient/families at established intervals, building repoire and education as needed.
Collaborates with referral partners, families and patients to provide direction for care
Maintains data metrics and reports to Elara Caring's leadership metrics of conversion to hospice, skilled home health, and palliative care
Communicates with families and/or patient on services offered through Elara Caring
Collaborates, supports and may backfill duties within the Intake department to initiate care.
Understands teamwork is a crucial part of the business and can work well with colleagues, no matter the circumstances. Uses individual skills, cooperates with others, and accepts and provides constructive feedback despite any personal conflicts between individuals involved.
Other duties and projects as necessary.
What is Required?
Graduate of an accredited Diploma, Associate or Baccalaureate school of Nursing. A degree in a related field such as Healthcare Management or Sales and Marketing may also be considered.
Current state license or Compact license RN.
3+ years practical experience in case management, clinical sales, hospice evaluations and/or admissions.
Proficiency with computer systems including MS Office Suite and/or EMR systems.
Ability and willingness to travel approximately 20% to meet business needs.
Ability to work remotely in a HIPPA compliant home office.
Demonstrates effective written and verbal communication skills.
You will report to the Director of Patient Access.
