Racker Self Directed Community Support Professional in Cortland, New York

Come join Racker as a Self Directed Community Support Professional where you can provide community based services for individuals with developmental disabilities and their families.

The Self Directed Community Support Professional works in accordance with service plans and individual interests of the people they support. The positioned is co-managed by the service recipient and their family along with the employing agency.

Hourly Rate: $18.45 - $25.41*

*Higher rates are possible depending on the candidate and service receipient meeting certain criteria. The team at Racker will review rate expectations prior to offering the position.

Responsibilities

  • Responsible for the health and safety of the individuals supported

  • Provide individualized services to people with disabilities and their families which may include: respite for families, teaching, modeling, and supporting skill development in a variety of areas at home and in the community

  • Participate as an active member of the person’s team providing ideas, input, and advocacy for the person related to his or her Staff Action Plan

  • Follow objectives as described in the person’s Staff Action Plan and as discussed with the person receiving services and their Team

  • Attend training to improve services provided

  • Complete job requirements (training, PPD, etc ) within designated time frames

  • Report any potential incidents to the supervising/assigned Fiscal Intermediary, or the Self-Direction Coordinator

  • Accurately enter time and services into Racker’s electronic health record, eVero , documenting service times, supports provided, and mileage according to program and regulatory requirements (e.g., contemporaneously documenting service provision)

  • Provide and use a smartphone for required documentation and other assigned tasks. (Ownership/usage of personal smartphone is strongly preferred)

  • Provide input concerning goals and service of the individual receiving support

  • Communicate regularly with the person being supported and their team regarding service provision

  • Uphold standards of professionalism with regard to behavior and dress

  • Assume responsibility for related duties as required or assigned

    Working Relationships:

  • Directly responsible to the person receiving services, their identified designee/family, AND the assigned Fiscal Intermediary

  • Member of the team which may include: the person and his or her family members, the person’s circle of support, the Care Manager, Support Broker, Fiscal Intermediary, teachers, therapists and other providers as selected by the person receiving services

Qualifications

  • High school diploma or GED

  • Must meet Approved Driver Status
