Racker Family Support Coordinator and Library Support in Cortland, New York

Rate: $18.60-$20.60 per hour
Responsibilities
Primary: Family Support Coordinator
Ensure families receive timely, high-quality services and support.
Act as a liaison between individuals/families, community support professionals, and Care Managers.
Assess individual and family needs, initiate appropriate services, and provide referrals as necessary.
Develop and oversee Staff Action Plans in collaboration with families, Care Managers, and community professionals.
Recruit, train, and supervise Community Support Professionals, ensuring quality services and credential compliance.
Conduct performance evaluations and facilitate staff development opportunities.
Maintain confidential records for individuals and staff, ensuring compliance with documentation standards.
Promote public awareness of community support programs and encourage community inclusion for service participants.
Complete monthly reports and financial/usage reviews as required.
Secondary: Library Support Specialist
Oversee daily operations of Racker’s Lending Library, ensuring it is safe, organized, and accessible.
Provide customer-focused support to individuals and families seeking library services.
Maintain library inventory, including inspecting, sanitizing, and organizing materials.
Collaborate with volunteers and other programs to deliver library services effectively.
Prepare service documentation and quarterly summaries for reporting purposes.
Ensure compliance with OPWDD (Office for People with Developmental Disabilities) guidelines and administrative requirements.
Qualifications
Education/Experience:Bachelor’s degree preferred, or a combination of education and relevant experience. Minimum of one year working with individuals with developmental disabilities.
Skills/Abilities:
Excellent interpersonal, communication, and organizational skills.
Strong writing and computer skills.
Ability to prioritize, multitask, and work both independently and collaboratively.
Valid driver’s license and dependable transportation.
Knowledge:Familiarity with developmental disabilities, human development, family dynamics, and local community resources.
