$31.78-$39.57/hour - Placement in the range will be determined based on experience and other factors allowed by law.

GENERAL SUMMARY Promote, coordinate, develop and oversee all aspects of Day Habilitation and Respite Day Hab Services through the Office of People with Developmental Disabilities. Oversee the delivery of service, supervision of staff and compliance with OPWDD regulations.

ESSENTIAL FUNCTIONS Ensures that supervisors and team leaders are coordinating and maintaining workload and work assignments for day habilitation and respite Day Hab employees such that program and individual needs are covered on a daily basis. Provides guidance and assistance, as needed. Oversees the process of staff and team assignments to ensure that safeguards and safety requirements are effectively delivered and maintained. Back up to the supervisors for attending life plan meetings for individuals served and completion of required staff actions plans and documentation. Ensures that supervisors follow up on and complete all required documentation regarding the services provided to each individual, as required by meeting outcomes. Oversees service delivery by all employees to ensure timely and compliant documentation is received for billing, and that records and documentation meet regulations and departmental policies. Designated contact and liaison for First Transit and all transportation providers. Receives and follows up on all transportation concerns, and elevates serious concerns to the Sr VP of Services. Ensures that all company vehicles, program equipment and the physical plant are in good repair, maintained and clean. Immediately reports any mechanical, safety or other needs for repair and maintenance to the Facilities Maintenance Director of designee. Coordinates the intake activities as the Point of Contact for the day habilitation program, and Respite Day Hab. Ensures that all required documentation and assessments, including those required for high needs funding is completed.
Responsible for oversight of activities, clinical and consultant services to ensure they are provided and according to individual needs. Responsible for day-to-day direction of nursing services, including but not limited to ensuring that all nursing plans and documents are completed in a timely manner and are consistent with OPWDD regulations and requirements. In collaboration with the Director for Behavioral Support and Community Services Responsible for day-to-day direction of behavioral services, including but not limited to ensuring that all support plans are completed in a timely manner and are consistent with individual needs.
Communicates and works cooperatively with consultants, residential staff, care coordinators, family members and others, ensuring services are integrated and necessary supports are provided (e.g., adaptive equipment, transportation arrangements are made, etc.). Ensures follow up and timely response on all compliance audit findings to the Sr VP of Services and the JM Murray Compliance Department. Provides timely responses to reported incidents and investigations by ensuring the appropriate safeguards are immediately implemented and communicated and promptly follows up on all investigation recommendations. Other services as directed.

EDUCATION and EXPERIENCE Bachelors Degree in human services, psychology or related field Must have 3-5 years of experience in working with individuals with disabilities, and 3-5 years of experience in successively responsible supervisory/management roles; Associates degree and significant experience in similar position may be considered in lieu of education requirement.
