Family Experience: https://www.youtube.com/watch?v=hIGQYYVzDDQ Partners For Kids Website: partnersforkids.org The Care Coordination Nurse ensures quality patient care in the appropriate setting through care coordination, case management, coordination of hospice and home-based palliative care admissions, and transitions to different levels of care. The Care Coordination Nurse collaborates to organize individualized plans of care and communicates the plan to appropriate stakeholders. Practices professional nursing and promotes patient advocacy in accordance with the ANA Code of Ethics for Nurses. Maintains a current Ohio Nursing License. Why Nationwide Children's Hospital? Responsibilities Assesses existing or incoming referral patient/family strengths and needs. Develops a family-centered plan of care as a member of the interdisciplinary team and as requested by the hospice program manager. Manages transitions between settings, caregivers, and providers. Facilitates communication and collaboration amongst the health care team, supporting the complex needs of hospice and home-based palliative care patients and ensuring the most seamless experience as possible. Attends and coordinates inpatient care conferences for home-based patients as requested by the attending provider or hospice program manager. Assists in educating others regarding the role of care coordination/case management. Participates in ongoing education related to information technology requirements for the department. Qualifications Education: BSN Required Licensure Requirement: Current licensure as a registered nurse in the state of Ohio is required. Valid Ohio drivers license and proof of auto insurance as required by hospital policy. Must pass motor vehicle background inspection, insurance eligibility, driving qualifications, and training set forth by Nationwide Children?s Hospital and maintain qualification of insurance guidelines Certifications: Pediatric certification, Advanced-Continuity of Care Certification (A-CCC), or other Case Management certi
