Additional information will be provided by HR upon hire. Planned Parenthood of Greater Ohio is seeking a mission-minded Associates to join us as a full time (37.5 per week) Patient Navigator. SUMMARY The Patient Navigator provides case management and care coordination services to the patients of PPGOH, by collaborating with internal and external teams that provide healthcare and non-health services within the medical neighborhood. This would include but not be limited to PPGOH health center staff and providers, customer contact center, centralized follow-up, billing, education and outreach, as well as primary care providers, hospitals, behavioral health providers, other specialists such as OB/GYN Specialists, community organizations, and insurance/payers. The Patient Navigator supports the PPGOH Care Coordination team to continuously build out and identify patient needs in the medical neighborhood to ensure our ability to provide wrap around care. The Patient Navigator also supports community outreach efforts in collaboration with the Director of Care Coordination and Referral Resource Manager. The Patient Navigator contributes to PPGOH's mission of providing high quality, equitable, and patient-centered care, as well as creating an outstanding patient experience. Additionally, the Patient Navigator ensures the organization is compliant with fulfilling these duties in line with the parameters set forth in the organization's Medical Standards and Guidelines, PPGOH policies and procedures, and any other regulatory requirements. Provides case management to patients with healthcare needs including but not limited to: OB/GYN services, Imaging services, Primary Care, Hypertension, Diabetes and acute and chronic disease management, GAC, Tobacco Cessation, Behavioral Health, and PrEP and PEP services. Systematically prioritizes case management by equitably identifying and directing resources based on need to prevent poor outcomes. Connects patients to resources and services in the community to address social determinants of health.
