Job Summary: In accordance with the organization’s mission, vision and values, the LPN is responsible for the general medical administrative & programmatic oversight of all clients as it relates to medical & behavioral health services.

Minimum Qualifications: Valid NYS LPN. Valid NYS Drivers’ License. Behavioral Health background preferred. CPR Certification will be required within thirty days of hire.

Job Requirements:

Duties:

  1. Coordinate and document medical program compliance in accordance with Agency & OASAS policies, procedures and regulations.

  2. Provide available support to residents and on site daily for oversight and documentation of self medication.

  3. Ensures that all documentation for medical appointments is recorded accurately; track outcomes & needs.

  4. Completes and monitors medication administration records.

  5. Assists with program applicant referrals, screens, admissions and discharges in a professional and proficient manner.

  6. Obtains and maintains knowledge of community resources to coordinate or refer clients to a full range of medical services.

  7. Participate in general, staff, in-service and supervisory meetings held by the Agency.

  8. Participates in agency/program continuous improvement/department enhancements related to medical/nursing.

  9. Provides program manager, LPN and counselors with verbal communication updates of client/program/staff status at each shift change or more frequently as necessary.

  10. Provides programmatic oversight of the development and implementation of clients’ objectives as it relates to medication management and/or medical issues. Provides instruction and/or education to clients as it pertains to medication management and/or medical issues. Ensure documentation of clients’ progress is complete, accurate and timely.

  11. Provide direction, and assistance to program staff where there are medical concerns.

  12. Responds appropriately to crisis and emergency situations in order to ensure/secure the health and safety of the clients. Adheres to regulatory guidelines and follows-up by appropriate communication and documentation to identified personnel.

  13. Assures that all clients’ required record documentation is accurate and timely and in compliance with Agency and regulatory policies, practices and procedures.

  14. Perform other duties and functions as directed.

Other Skills/Knowledge and Experience

  1. Excellent written and verbal communication skills.

  2. Demonstrated ability to communicate effectively and work cooperatively with culturally diverse persons, staff and community service providers.

  3. Working knowledge of behavioral health.

  4. Ability to multi-task, have good problem solving & time management skills.

  5. Recognition and understanding of the scope of practice according to the Nurse Practice Act.
