Ohio Means Jobs Cleveland Cuyahoga County is sourcing for a Staff Guardian position. A Staff Guardian serves as the legally appointed Guardian of Person for incompetent adults and acts in accord with their best interest as an advocate and substitute decision-maker in the care and management of their ward's personal and/or financial affairs. Guardianship Services provides services to residents of Cuyahoga and Lorain counties. Essential Job Responsibilities ? Manages a full caseload of wards ? Regularly visits with wards. Routinely requires visits to 4-6 different community locations weekly. ? Contacts/intervenes with family members and other service providers ? Assists with benefits and other entitlements for which the ward needs and is eligible ? Makes decisions (e.g. appropriate living arrangement, medical care, funeral arrangements) for ward as required ? Communicates all major life decisions regarding wards to supervisors, prior to acting on the decisions (i.e. move from nursing home to community, major medical decision) ? Attend guardianship hearings at Cuyahoga and/or Lorain County Probate Court, as needed ? Prepares and maintains, through the database, a record of all work-related activities and contacts performed on behalf of the ward ? Prepares all documents required by Probate Court ? Maintains a working knowledge of community resources ? Is available, via cell phone, 24 hours/day, seven days/week Other Job Responsibilities • Develops productive working relationships with all staff, Probate Court, and service providers ? Attends staff meetings and other training, as required ? Engages actively in regular supervision • Completes incident reports, as needed • Complies with the policies and procedures set forth by the organization • Performs other duties as assigned that support the mission of the program/agency • Actively participates in quality improvement activities of the program/agency Required Knowledge, Skills and Abilities* • Communicates effectively (written and oral) • Comfortable asserting self in role of advocate • Works compassionately with difficult clients and situations • Extremely detail-oriented • Computer knowledge including Microsoft Office applications and internet • Must have reliable transportation to locations that may or may not be located on public transportation lines to provide services to wards in the community • Must be able to get into and out of buildings which may not be handicap accessible *NOTE: The individual must possess these skills and abilities or, using some other combination of skills and abilities, be able to explain and/or demonstrate that he/she can perform the essential responsibilities of the job with or without reasonable accommodations. Qualification Standards • Bachelor's degree preferred in health and human services, or equivalent combination of education, training, and experience • 2-5 years' experience serving the severely mentally disabled, the elderly, or other vulnerable populations • Knowledge of community-based resources • Additional experience in fields of long-term care, dementia and related illnesses, substance abuse, traumatic brain injury, and related areas desirable • Must not have been found guilty of a felony in Ohio (court requirement) Interested candidates please contact: LaShorn Caldwell-Recruiter lcaldwell@dwfs.us 216-777-8227
