Social Worker Manager
[General Purpose:]{.underline}
To provide oversight of the social work team to ensure the necessary services are provided for each resident to maintain his/her optimal level of psychosocial well-being.

[Leadership Role Responsibilities]{.underline}
- Serves as a working manager to provide social work leadership across the organization while maintaining a caseload of core social work responsibilities
- Provides support and education to the social work team to ensure that ongoing services are provided across campuses
- Collaborates with other members of the leadership team to identify gaps or areas of improvement for social work services
- Establishes departmental goals and objectives in alignment with strategic priorities
[Social Work Responsibilities]{.underline}
- Provide ongoing casework, including but not limited to, interviews, crisis intervention, grief counseling,
psychosocial assessments, information and referral, discharge planning, suicide risk assessments and advocacy.
- Responsible for completing social history and updating resident information
- Provide one on one counseling for residents/families/staff.
- Participate in interdisciplinary care planning and complete quarterly care plan documentation and progress notes in the medical record to meet licensure requirements and professional social work standards.
- Complete necessary MDS assessments and maintain tracking documentation.
- Assist residents/families with application and eligibility for Medicaid and other assistance programs.
- Ensure that the PASRR screens, Pas Ids and levels of care are completed in a timely manner and placed in the correct section of the resident's medical record.
- Prepare information for submission to Medical advisory committee.
- Conduct family meetings to prepare resident/family for level of care change.
- Conduct such clinical audits as is deemed appropriate by the director.
- Attend and participate in weekly, monthly and quarterly administrative meetings, standing or Ad Hoc committees as pertinent to the position.
- Maintain resident social work records and files.
- Provide appropriate coverage for colleagues when needed.
- Work as part of social work team and interdisciplinary team to accomplish goals of the department and the organization.
- Participates in the establishment of departmental goals.
- Attend and participate in continuing education programs and all mandatory in-services
- Maintain a current working knowledge of Medicare/Medicaid/Social security and other benefits.
- Maintain working knowledge of the Probate court system and the various community- based agencies that could be of benefit to residents.
- Follows established Infectious Control and Universal Precautions policies and procedures.

``` - Reports all unsafe/hazardous conditions, defective equipment etc to appropriate departments. - Comply with and be able to explain HIPAA and Corporate compliance regulations. - Provide educational seminars/in-services for staff/residents as required. - May provide educational presentations for outside groups. - Miscellaneous: Maintains a record of acceptable attendance and punctuality. Observes all Judson rules and safety procedures. Other duties as assigned.
