Description Position Summary/Essential Duties: * Provides assistance/support for Pediatric Diabetes patients and families guiding them in identification/navigation of services available with UHHS and in the community. * Provide competent, efficient and caring services to Pediatric Diabetes patients and families. * Develop effective working relationships with physicians, nurses and other Pediatric Diabetes team members. * Primary support person working with Pediatric Diabetes Nurses on pre-authorization process, referral processes and other similar tasks for the Pediatric Diabetes patients and families. * Conduct face-to-face/virtual discussions with patients and families in order to schedule appointments and reduce no-show rates. * Takes responsibility for self-development and supports a learning environment. * Performs other duties as assigned.
