Job Summary:

At LIFE-NWPA, our Clinic Nurse Manager role is responsible for all clinical discipline and services provided through the Center and administrative tasks related to the direct operation of the clinic.

 

Schedule:

Standard operating hours are 8:00 am -- 5:00 pm; Monday-Friday. Individual hours will be based on discussion with supervisor.

 

Salary Wage:

Starting at $83,000 Annually

 

Duties/Responsibilities:

  • Responsible for management of all clinical disciplines within the Center.
  • Coordinates all clinical activities, including care planning and implementation, preparation for the interdisciplinary conference and daily meetings, family conferences, and all related follow-ups.
  • Supports the Center Manager in creating a strong community presence through relationship building, outreach and collaboration with local service providers, vendors, and potential sources of Participant enrollment referrals.
  • Serves as liaison to Center Manager, Quality Improvement Director for proper audits and implementation of Plans of Correction when required and demonstrates active participation in QI processes.
  • Responsible for training and competencies completion.
  • Participates as a member of the Center Management Team and serves as a liaison between administration and direct care staff.
  • Coordinates all clinical activities relating to new enrollees.
  • Conducts or delegates initial and periodic assessments.
  • Communicates participant changes to team members.
  • Participates in the development of the IDT Plan of Care.
  • Participates in the coordination of a 24-hour care delivery.
  • Assures all documentation in the electronic medical systems record as required.
  • Directly fills in for vacation, call-offs, during growth periods, and/or openings for other RN staff; if properly licensed to do so or under the supervision if required for the scope of the license held.
  • Provides on-call support in rotation with other Clinics; if properly licensed to do so for the scope of the license held.
  • Other duties as assigned.

     

Supervisory Responsibilities:

  • The Clinic Nurse Manager oversees clinical disciplines within the Center, including RNs, Medical Records, and Clinic Assistants.

     

Required Skills/Abilities:

  • Must be able to deal tactfully and patiently with personnel, Participants, family members, visitors, government agencies/personnel, and the public.
  • Communicates with the medical staff, nursing personnel, and other department supervisors.
  • Must possess the ability to make independent decisions when circumstances warrant s ch action.
  • Ability to organize work and follow through on assigned tasks.
  • Excellent written and verbal communication skills.
  • Must have the cognitive ability to process and perform tasks for Participant safety.
  • Attends and participates in continuing educational programs.

     

Education and Experience:

  • One year of experience working with the frail and elderly population or willing to participate in extensive training as needed.
  • Must possess a current state license to practice nursing and a valid driver's license.
  • Able to function well in a team environment.
  • Possess good organizational, communication, and supervisory skills.
  • Able to effectively handle difficult and unusual interpersonal situations.

     

Requirements:

· Successful completion of background check, physical, drug screening, TB Screening

 

Physical Requirements:

  • Must be able to move intermittently throughout the workday.
  • Must be able to drive a motor vehicle.
  • Sits, stands, bends, lifts, and moves intermittently during working hours.
  • Occasional overnight or day travel for training, continuing education, business meetings, etc.
  • Is subject to lifting, carrying, and supporting Participants.

Employment Requirements: Successful completion of Pennsylvania Criminal History Check, PA DHS Child Abuse Clearance, Office of Inspector General check, and FBI Fingerprinting for Child Protective Services Law (at employer's expense). Successful completion of post-offer employment drug screening and physical (at employer's expense). CPR and First Aid Certification, or willing to obtain at employer's expense. Individuals must have one year of prior experience caring for the frail elderly or willing to complete Company paid training to meet this requirement.
