Salary: Hourly Pay + Pay Per Visit (PPV) Position: Certified Occupational Therapist Assistant (COTA) Service Area: Northeast metro (coverage area: White Bear Lake, Stacy, North Branch, Forest Lake area and Surrounding). Plus mileage reimbursement of .67 cents per mile. The Certified Occupational Therapist Assistant is primarily responsible to the registered occupational therapist, who is responsible for the implementation of standards of care for occupational therapy services and for adherence to all conditions outlined in the service agreement. The certified occupational therapy assistant performs only those services planned, delegated, and supervised by the registered occupational therapist. Position Functions and Responsibilities: Participates in the ongoing evaluation of client's functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.) as delegated by the registered occupational therapist. Participates in the ongoing evaluation of the home environment for hazards or barriers to more independent living as delegated by the registered occupational therapist. Participates in teaching new skills or retraining clients in once familiar daily activities that have been lost due to illness or injury, in accordance with organization policy. Maintains appropriate clinical records, clinical notes, and reports to the registered occupational therapist any changes in the client's condition. Submits clinical documentation in accordance with policy. Follows treatment program and goals for improved client function as established by the registered occupational therapist. Documents client's response to treatment plan and progress toward established goals. Maintains contact/communication with other personnel involved in the client's care to promote coordinated, efficient care. Documents such communication in accordance with organization policy. Attends and contributes to inservices, case conferences, and other meetings as required by organization policy to ensure coordinated and comprehensive plans of care for the clients of the organization. Identifies client and family/caregiver needs for other home care services. Consults with the supervising registered occupational therapist and assists with necessary referrals, as appropriate. Participates in instructing client's famil /caregiver and other organization health care personnel in client's treatment regime as delegated by the registered occupational therapist. Is supervised by the registered occupational therapist no less than every thirty days. Documentation in the clinical record will reflect ongoing communication between the registered occupational therapist and certified occupational therapy assistant, the patient's condition, the patient's response to services provided by the assistant, any need to change the plan of care, and patient outcomes. Additional Client Care & Service Delivery as needed: Provides personal care including: Baths Back rubs Oral hygiene Shampoos Changing bed linen Assisting the client with dressing and undressing Skin care to prevent breakdown Assisting the client with toileting activities Keeping client's living area clean and orderly Planning and preparing nutritious meals. Assisting in feeding the client, if necessary. Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency). Assisting in ambulation and exercise according to the plan of care. Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency). Assisting client in the self-administration of medication. Doing client's laundry, as appropriate. Meeting safety needs of clients and using equipment safely and properly (foot stools, side rails, etc.) Reporting on client's condition and significant changes to the assigned supervisory nurse. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. Lead by example in establishing rapport with clients. Ensure positive/professional communication on team. Customer service resolution; escalate concerns. Honors client confidentiality, rights, privacy, and reporting maltreatment expectations. Understands and accepts emotional needs of client. Accepts unique client symptoms and behaviors. Follow all Emergency and Response Procedures Identifies home safety issues and notifies RN Case Manager or Clinical Manager of concerns or issues Demonstrates proper procedures: Handling, lifting, transfers Injury - Now What? Follows protocols Overview of Process, Types of Reports and Forms Follow-up Required - Escalating Event Other duties as assigned Qualifications: A minimum of one year of clinical experience as a Certified Occupational Therapist Assistant. Graduate of an occupational therapy assistant curriculum accredited by the American Occupational Therapy Association. Current and active Occupational Therapist Assistant license in the state of Minnesota. Experience using technology including: Microsoft Office, Smart Phone, Calendaring, Clinical Software platforms (EMR), Wi-Fi, Printers, and Internet Browsing Ability to follow written instructions and type a minimum of 20 words a minute.
