Location: UCMC Department: Care Management Hours: Full-Time, 40 Hours/Week Shift: First, 8:30am-5:00pm & 1 holiday per year Discharge Planning Assistant, Full-Time, First Shift, Care Management UC Health is hiring a full-time Discharge Planning Assistant for the care management department for the first shift at University of Cincinnati Medical Center. The Care Management Discharge Planning Assistant will support the Medical Center's Care Management department, delivering high-quality care to patients based on assessed needs, established standards of care and according to policy and procedures. Education and Experience Requirements:
- Education: High school diploma or GED (required), bachelor's degree in social work or nursing (preferred)
- Experience: 1 to 2 years (preferred)
