Wellness Health Center Nurse DEPARTMENT: Council on Aging SUMMARY: Oversees the services of the Wellness Health Center. Facilitates the diabetic elders support groups. Responsible for all record keeping and follow-ups. Maintains supplies for the Wellness Center. Conducts cholesterol screening six times per year. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned: Provides initial assessment to Chicopee residents who are sixty (60) years or older. Follows-up, monitors and make referrals as necessary. Counsel clients and/or family regarding health care needs and problems. Tests for diabetes, high blood pressure, anemia, weight loss or gain or depression as necessary. Conduct bi-monthly cholesterol screenings. Informs elders on proper medication management and unhealthy use of alcohol. Keeps accurate health records on each client. Reviews with the Health and Fitness Coordinator all forms necessary for participants of the Fitness Center and Programs. Performs physical exams for all Chicopee Senior Companions. Supervises any student nurse placed in the Wellness Center Facilitates a support group for diabetics. Meets with Wellness Team to develop appropriate services for clients. Other duties as assigned by the Social Service Coordinator. Current Massachusetts Nursing License, and at least five (5) year direct care nursing with seniors. LANGUAGE SKILLS: Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from clients, regulatory agencies, or members of the business community. Ability to write articles for publication that conform to prescribed style and format. Ability to effectively present information to top management, public groups, and/or boards of directors. MATHEMATICAL SKILLS: Ability to work with mathematical concepts such as statistical inference. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. REASONING ABILITY: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
