On an Average Day, you will:

  • P lans, directs, and participates in delivery of home health services to provide diagnosis-specificpatient care, including coordination of services with providers, vendors, or facilities.

  • P rovides oversight of allpatient careservices and personnel. Oversight includes making patient and personnel assignments, coordinatingpatient care, coordinating referrals, assuring patient needs are continually assessed, and assuring the development, implementation, and updates to the individualized patient plan of care.

  • P lans, directs, and participates in delivery of home health services to provide diagnosis-specificpatient care, including coordination of services with providers, vendors, or facilities.

  • E nsures the timely start ofpatient carewithin recommended regulatory guidelines.

  • P rocesses and coordinates physician orders, staff assignments, and communications between direct care team members, the patient and their family, the following physician andprimary carephysician (as appropriate).

  • R eviews and approves plan of care and evaluates proposed changes to the plan of care for clinical appropriateness.

  • I ntegrates orders from all relevant physicians involved into the plan of care and ensures the orders are approved by the responsible physician.

What is Required?

  • Associates Degree in Nursing related field is required.

  • 2+ yearshome careexperience as a Registered Nurse in Home Health orBehavioral Health

  • 1+ year supervisory experience

  • Current state license as a Registered Nurse

  • Proficiency withenterprise applicationssuch as Workday and HomeCare HomeBase

  • Proficiency with MS Office Suite (Word, Excel, Outlook)

You will report to the Branch Director.
