Cascade Community Healthcare Care Coordinator in Centralia, Washington

ABOUT CASCADE:

Cascade Community Healthcare (CCH) has been providing treatment services to Lewis County and surrounding areas for more than 50 years. Our mission is to provide our clients with compassionate and encouraging counseling leading to a renewed sense of hope and the ability to make positive changes and live more satisfying lives. Cascade currently has six treatment locations in Lewis County.

 

We are looking for a Care Coordinator to join our Child and Adolscent Program (CAP). You will provide coordination between community members and Cascade Community Healthcare (CCH) clients who have been identified as needing assistance with accessing and utilizing the health care/behavioral health care system. This position will work collaboratively with advocates, clinicians, and peer supports to provide support and assistance.

 

SHIFTS AVAILABLE:

Day Shift, Monday - Friday

 

This position will normally require that the employee work a standard, Monday through Friday, forty (40) hour work week.  This position is classified as being "non-exempt" according to the Fair Labor Standards Act.

 

PAY SCALE:

$18.90 - $23.04 Hourly, DOE.

 

WHAT YOU NEED TO GET THE JOB DONE:

  • Registered Agency Affiliated Counselor (RAAC) credential required, or eligibility to obtain RAAC credential.
  • Must be certified as a Peer Counselor, Peer Counselor Certification must be recognized by Washington State.
  • Bilingual preferred (fluent in English/Spanish).
  • Ability to interact with and engage clients effectively.
  • Must be able to pass WA State Patrol background check. A national background check may be required if haven't lived in WA state consecutively the last three years.
  • Must be able to represent the agency in a positive and professional manner.
  • Must be able to communicate clearly, both verbally and in writing.
  • Must be able to enter information into a computer network system.
  • Must comply with agency policies and procedures.
  • Must have a valid driver's license.
  • Must be able to operate a motor vehicle.
  • Must be able to gain access to all types of buildings and residences regardless of the structure's handicapped accessibility or lack thereof.
  • Must maintain current phone number and mailing address on file with the human resources office.

     

WHAT YOU'LL DO:

  • Initiate contact with clients and helps with accessing services as needed.
  • Utilize various reports to identify and track clients who haven't followed through with primary care referrals.
  • Coordinate with Community Providers and CCH as needed to engage clients.

``` - Coordinates with Community Providers and CCH personnel to ensure effective client care. - Meets productivity/direct service requirements as assigned by the program manager/director. - Maintains, records and reports in accordance with agency policies and procedures. - Participates in supervision, consultation, and training at the direction of the Program Manager. - Must be able to complete all paperwork associated with position according to agency policy and procedures. - Other duties as assigned by program manager/director.
