Long Island Select Healthcare Inc. (LISH) is a comprehensive network of community health centers under a Federally Qualified Health Center (FQHC) model. We provide Article 16 and 28 clinic services at 3 locations across Suffolk County, New York. LISH is currently searching for a Licensed Clinical Social Worker (LCSW) who will report to the Lead LCSW. We are looking for compassionate, energetic individuals who will have the opportunity to work alongside our experienced clinical staff. This position is Full-Time.

General Purpose of the JobProvides individual, marital, family and group psychotherapy, as referred.II. Essential Duties & Responsibilities1. Provides psychosocial assessments as referred.2. Provides mental health intake assessments as referred.3. Maintains close liaison with other care providers, especially psychiatrists involved in patient’streatment.4. Prepares Initial Assessments, Treatment Care Plans, Treatment Care Plan Updates, daily notes,discharge notes, etc., per Agency policy, in a timely manner.5. Will provide relevant information about the PCMH program including brochures, that describethe values and goals of the program to patients. The information disseminated will be based uponthe person’s role as it relates to patient care within LISH6. Attends necessary in-service education programs.7. Means and ability to travel between Agency buildings8. Maintain licensing status in NYS by completing 36 hours of continuing education (CE) from aNY Department approved provider during each three-year registration period.III. Education and/or ExperienceMaster’s Degree from an accredited School of Social Work in the United StatesMinimum 2 year’s clinical experienceNYS Licensed Clinical Social Worker

Maintain licensing status in NYS by completing 36 hours of continuing education (CE) from a NYS Department-approved provider during each three-year registration period.

QUALIFICATIONS:

  • Graduated from a regionally accredited school

  • Licensed & Registered to practice in NY by the NYS Department of Education

  • Basic Life Support Certification Required

  • Bilingual in Spanish/English Preferred

  • Ability and means to travel to other sites for coverage as needed

  • Rate of pay for this position starts at $39 per hour, negotiable dependent upon experience
