Why This Role?

  • Competitive Pay: $21/hour at up to 18 hours per week. Hours are flexible, evenings and weekends required

  • Location: Central Islip, NY, with travel throughout Suffolk County for home visits.

    Your Mission:As a Bilingual Case Manager, you’ll provide hands-on, home-based support to both English- and Spanish-speaking families dealing with crises like substance abuse, mental illness, and behavioral challenges. Your goal? Help them find stability and avoid the need for legal interventions.

    What You’ll Do:

  • Assist Families in Crisis: Work closely with families to resolve issues that led them to seek help through a PINS petition (Person in Need of Supervision), offering support for substance abuse, mental health, and behavioral problems.

  • Provide Intensive Case Management: Conduct home-based visits, assess needs using the Child and Adult Needs Assessment (CANS), and create personalized Family Intervention Plans.

  • Crisis Intervention: Be the first line of support, offering crisis intervention to guide families toward stability.

  • Connect Families with Resources: Make referrals to partner agencies and find new resources when necessary. Follow up to ensure progress is being made.

  • Teach Behavior Management: Help parents and youth develop positive behavior management strategies to foster a healthy home environment.

  • Stay Organized: Maintain detailed case records and progress notes, ensuring everything is updated in the system on time.

  • Be on Call: Respond to families reaching out via the AFY hotline during scheduled times.

  • Help with Logistics: Provide transportation for clients when needed for emergency resources like clothing or medication.

  • Lead Workshops: Develop and facilitate monthly educational workshops for adolescents, providing life skills and guidance.

  • Participate in Youth Events: Get involved in monthly recreational activities and lead weekly adolescent groups for both girls and boys.

    What we are looking for:

  • Education: Bachelor’s Degree with at least one year of experience working with children and adolescents, or at least one year in community/social service work.

  • Valid Driver’s License: You’ll need your own vehicle for client visits and transport.

  • Tech-Savvy: Comfortable using Microsoft Office, email, and databases to manage case records.

  • Ongoing Learning: Participate in 20 hours of training during your first 6 months, with another 20 hours in the next 6 months. Training covers areas like mediation, substance abuse, mental health, and juvenile justice.
