Case Manager (CV42-2) - Homeless Services Division
We are looking for a Case Manager to join our dynamic and growing team. The successful candidate will report to the Director/Treatment Coordinator and will be a vital part of their Treatment Team.
JOB RESPONSIBILITIES:
Use advanced tools to conduct risk/needs assessments and develop service plans for all clients assigned to your caseload.
Implement those plans subject to review by the Treatment Team and/or approval of the Director/Treatment Coordinator.
Schedule and facilitate case management meetings with clients
Facilitate scheduled workshops, programs, and groups addressing clients' needs.
Maintain appropriate contact with outside agencies contracted to provide specific outsourced services.
REQUIREMENTS:
Strong interpersonal skills
Work well in a team.
The capacity to achieve an effective balance between set operational policies and procedures and the use of initiative as and when the need arises
Possess effective written and verbal communication skills
Have an ability to interact appropriately with clientele
Basic computer skills will be preferred
Must have a valid driver's license and meet VOADV agency standards
EDUCATION:
Minimum of a Bachelor's Degree
