Job Description:

Sign on bonus up to $3000 (pro-rated for part time)

Schedule: M-F 7a-3p or 8a-4p

Responsible for care coordination and monitoring referrals for behavioral health patients at hospitals served. Responsible for monitoring and assisting with mandatory hospital reporting requirements. Works closely with program management to ensure quality of care and procedural functioning of the program.

Responsibilities:

  1. Provides procedural direction and support to crisis clinicians by phone, chat, and/or face to face as needed.

  2. Responsible for the procedural functioning of the program. Monitors and facilitates mandatory hospital reporting requirements (MABHA, EPIA) for behavioral health patients at hospitals served.

  3. Maintains awareness of/monitors activity of active and pending cases.

  4. Manages daily tracking and coordination of CBHC, MCI and CCS referrals with local providers of hospitals served.

  5. Provides assistance to clinicians with bed searches, insurance verifications/authorizations, resource and system knowledge sharing.

  6. Serves as primary liaison between the organization and other providers such as hospital emergency rooms to ensure quality of care.

  7. Acts as a conduit for the flow of information regarding programmatic updates and clinical procedural alerts.

  8. Accesses program management or administrator on call when needed.

  9. Collects and records data and statistics for various reporting needs of the program, assists program management with organizing daily huddles with hospitals served when needed.

  10. Provides feedback to program management regarding issues effecting performance.

  11. In the instance of high volume or low staffing, provides direct care via telehealth or in person to the hospitals served.

  12. Other related duties as assigned.

Qualifications:

  • Master’s degree required.

  • The individual must have experience and training in emergency psychiatric services working with adults, youth and their families, including training in de-escalation strategies and emergency services system of care.

  • A qualified candidate must achieve effective outcomes and demonstrate effective communication and organizational skills.

  • A working knowledge of computers and ability to complete documentation electronically is required. In addition the ability to complete written documentation timely and effectively.
