Job Description

Summary:

Has the responsibility of collaborating with members of the interdisciplinary team to coordinate care, facilitate utilization and resource management and discharge planning functions, working to achieve optimal outcomes for the patient in the acute care setting. Case management is a process that efficiently and effectively aligns patient needs/issues with resources to meet quality, clinical and cost outcomes.

Education And Credentials

Responsibilities:

Current New York State registration as a Registered Professional Nurse required. Bachelor of Science degree in Nursing required within five (5) years of hire to the position. Certified Case Manager (CCM) preferred. Employees in the job title prior to 3/29/10 are grandfathered and not required to obtain their BSN. Basic Life Support (BLS) required upon hire. Current NYS registration as a Registered Professional Nurse/RN Permit and American Heart Association-Basic Life Support (BLS) required upon hire and must be maintained thereafter.

Experience

Other information:

2 years of acute care Nursing Experience required. 2 years of experience in any combination of case management, home care and utilization review preferred. Knowledge of and experience with Interqual Criteria set and software systems preferred. Working knowledge of Microsoft Office Suite and Outlook required. Use of computer applications related to health care industry required. 2 years of experience in providing case management, care coordination, and supportive services to individuals at risk or living with HIV in an outpatient clinic. (Youth Link Only) Required. 1 year of experience in providing case management, care coordination and supportive services to LGNTQIA+ individuals in an outpatient clinic. (Youth Link Only) Preferred.
