Evergreen Health Navigation Assistant in Buffalo, New York

The Navigation Assistant will provide services across Niagara, Erie, Chautauqua, Cattaraugus, Allegany, Wyoming, Genesee, and Orleans County and is responsible for the following services conducts outreach and engagement efforts, assists patients in self-advocacy and self-management, accompanies patients on clinical and supportive service appointments, and assists the Case Manager and Health Experience Coordinator during specified interventions and health education sessions. The Navigation Assistant reports directly to the Supportive Services Director, who in conjunction with the Senior Director for Supportive Services, determines the specific responsibilities of the position. These responsibilities may be revised, as necessary, with proper notification and training provided, where appropriate.

Essentials Functions:

  • Assists with development of the annual outreach plan, which includes face-to-face meetings, tabling events, and other goals and objectives for reaching the target population.

  • Conducts outreach according to strategies identified in the annual plan, grant work plan and the enhanced outreach model.

  • Works with participants to link them to care coordination and HIV medical care; assists participants with appointment reminders when needed; accompanies participants to appointments if needed.

  • Reaches out to patients who are not engaging with the Case Manager and Health Experience Coordinator in order to re-engage them in care. This outreach could include activities such as sending letters, conducting home visits, or meeting patients at a medical appointment.

  • Positive attitude and ability to work well in a team environment.

  • Ability to utilize personal and/or public transportation for patient services activities including accompanying/transporting patients when necessary and attending outreach events.

  • Ability to handle Protected Health Information (PHI) in a manner consistent with the Health Insurance Portability and Accountability Act (HIPAA) of 1996.

  • Demonstrate personal integrity and responsibility.

  • Demonstrate an awareness of and sensitivity to the issues that underserved populations, people living with HIV and people who use drugs face, including health disparities that affect people of color at a disproportionate rate

  • Conducts program orientation with patients new to the program or those that are reluctant to accept services.

  • Documents all outreach and linkage activities on program forms and in Medent according to program procedures and within required time frames; completes all required reports.

  • Tracks progress in accomplishing work plan goals; identifies barriers where goals are not being met and communicates them to the Director.

  • Assists with production of promotional items, brochures, fliers and other program literature that support and enhance outreach strategies.

  • Develops and maintains relationships with other community organizations to increase opportunities to reach the target population.

  • Collaborate and support the team members to ensure a patient-focused approach to care

    Qualified candidate: will have a High School diploma or GED preferred, with personal or professional experience with HIV/aging communities. The qualified candidate must be able to communicate clearly and effectively in writing and verbally; Fluent in reading, writing, and speaking English (Spanish a plus). The qualified candidate must have excellent interpersonal and customer service skills.

    Job type: Full-time

    Required education/experience: High School diploma or G.E.D. preferred. Personal or professional experience with HIV/Aging communities; sensitivity to HIV/AIDS, addiction and LGBT issues, essential.
