Summary:

The primary responsibilities of this position are to support and facilitate relationships and communication with patients who seek emergency department care for primary care services and to link them with a primary care physician and the appropriate community resources. This position will be responsible for addressing barriers to accessing primary care to receive the appropriate level of care, improve compliance with preventive care and chronic care, while supporting the patient's self-management skills for their underlying health issues. The overall goal of this position will be improve the health of the individual as well as reducing preventable emergency department visits and/or potential hospital admission.

Responsibilities:

EDUCATION

  • Master of Social Work (MSW) from a school accredited by the Council on Social Work Education

  • Current NYS Master of Social Work license (LMSW) or current NYS Clinical Social Work license (LCSW) preferred

EXPERIENCE

  • Two (2) years of healthcare/community health/social services/human services preferred

  • Prior telephonic support and electronic medical record experience preferred

KNOWLEDGE, SKILL AND ABILITY

  • Strong customer service skills, and have the ability to quickly establish a rapport with the patient

  • Strong written, verbal and interpersonal communication skills

  • Working relationship/knowledge of health issues/barriers and the Program's service area

  • Ability to work and develop a positive rapport with patients, their families, personnel, and other agencies

  • Ability to effectively relate to patient's and life situations in a professional, supportive manner

  • Ability to facilitate linkages to appropriate community based organizations, primary care providers, programs, and services as needed by the patient

  • Computer skills, data entry experience, EMR experience preferred; willingness to learn new software applications

  • Ability to work independently

  • Ability to discern issues that need to be escalated to management

  • Accountable and results oriented

  • Ability to follow protocols and adhere standard workflows while managing competing priorities simultaneously

  • Detail oriented
