POSITION OVERVIEW:
The case manager will provide case management services to help connect and refer clients to necessary services in the community. This includes working collaboratively with on-site and offsite providers to assess the needs of the client and the client's family, when appropriate, and arranges, coordinates, monitors, evaluates, and advocates for services to meet the specific client's complex needs.Pays: $24.72 per hour
KEY ESSENTIAL FUNCTIONS:
Accept referrals from intake staff members for clients in the Reintegration phase of treatment.
Connect and/or refer clients to necessary services (i.e., financial benefits, health insurance, housing, mental health services, vocational activities, etc.) within the community and onsite
Work with other clinical team members to implement service plans to help clients maintain sobriety and other goals.
Provide daily living skills and other groups to clients. Develops and conducts group, individual and family counseling on a scheduled basis to caseload clients
Orients clients about program goals, objectives, rules and regulations, client's obligations, client's rights, and program operation
Demonstrates knowledge of group work management techniques and uses those techniques appropriately
Assist with housing transition when appropriate.
Complete assigned reports on time on EHR, STARS, etc.
Demonstrates knowledge of appropriate documentation
Provides education and information to the client about alcohol and drug abuse
Maintains and updates referral services and resources that may be useful to our client/resident population; makes referral when necessary and appropriate
Participate in multidisciplinary and other team meetings
Prepare monthly reports monthly or as needed
Other duties as assigned by the supervisor.
REQUIREMENTS:
HS. Diploma or equivalent required
Bachelor's degree in social work, Counseling, or any other social service-related field, preferred.
2+ years' experience working with individuals with co-occurring disorders, chemical dependency treatment services, and the criminal justice system.
Evening and weekend work activities will be required.
Must obtain Mandated reporter (2 hours) training/certificate.
