RiseBoro Homecare Inc. Program Coordinator, Social Adult Day Care in BROOKLYN, New York

ESSENTIAL DUTIES AND RESPONSIBILITIES

Under the Program Director's supervision, the Social Adult Day Care Program Coordinator will assist in the supervision, planning, and coordination of Social Adult Day services. Additionally, the coordinator will provide administrative support in maintaining services and billing documentation for client data. Specific duties and responsibilities include:

  • Track and maintain client attendance records in the NYC Aging Peerplace database for Social Adult Day and any other related or required documentation

  • Organize and conduct activities to meet the special needs of the SAD’s program participants

  • Assist members in the development of skills

  • Coordinate special programs and events as required

  • Conduct client intake, assessments, and reassessments as required by the designated funding source

  • Schedule Social Adult Day Services trials i.e. providing caregivers with the required admission paperwork and the appropriate physician’s assessment form

  • Record and maintain accurate daily RiseBoro program attendance records, client contribution records, and income records in Excel worksheets

  • Provide case assistance to caregivers which includes linkages to resources and information on benefits and entitlements

  • Conduct community outreach to establish and maintain partnerships to meet the goals of the program

  • Collaborate with HHA(s) to conduct daily program activities and update the monthly program calendar

  • Ongoing training and workshops and annual physical exam statement and PPD test required

  • Perform other program duties and HHA duties as required by the Program Director

    COMPETENCIES

  • Demonstrated experience working with the physical, mental, and emotional needs of older persons with cognitive impairment i.e., Dementia, etc.

  • Demonstrated work experience in a social service setting, and/or a senior center or social adult day setting preferred

  • Possess an energetic, enthusiastic attitude with an ability to build positive team spirit

  • Ability to show genuine interest in helping and improving the lives of older adults

  • Ability to provide quality services with compassion, patience, and empathy

  • Excellent verbal and written communication skills

  • Strong organizational and time management skills

  • Excellent interpersonal and customer service skills

  • Flexibility with program-related communication as needed i.e. morning, afternoons

  • A self-starter with the ability to work independently and collaboratively as a team

  • Commitment to RiseBoro’s mission, vision, and values

  • Knowledge of funding source databases i.e., PeerPlace preferred

  • Mental Health First Aid skills-based training course completion a plus

  • Proficient in Microsoft Office Suite, specifically Word, and Excel

  • Ability to understand the concepts of institutional and structural racism and bias and their impact on underserved and underrepresented communities

    Commitment to supporting communities who have experienced systemic oppression and bias (e.g. people of color, LGBTQ+ people , immigrants, justice-involved persons, etc.)

    EDUCATION and EXPERIENCE

  • Bachelor’s degree in human services preferred;

  • Associate degree with a minimum of one (1) year of experience in a social service setting, a senior center, a social adult day setting, and/or a related setting OR

  • High School Diploma or equivalent with a minimum of three (3) years of Certified HHA experience and or three (3) years in a social adult day setting, and/or a related setting
