Position Title: NHTD/TBI Case Manager

Job Description: As the Waiver Case Manager at True Care, you will guide prospective participants through the enrollment and onboarding processes for the NHTD/TBI Waiver program and support their ongoing needs with the home care provider. Your role provides prospective and active participant and family support with frequent communication and follow up, verification of eligibility for assistive programs and maintenance of accurate and comprehensive documentation. The goal of this position is to support each candidate’s enrollment in the NHTD/TBI Waiver program and maintain their independence in the community. By ensuring timely interventions, you will contribute to improved patient outcomes, reduced hospitalizations and the benefits of keeping people in their homes.

Responsibilities:

  • Act as an advocate for the prospective participant for NHTD/TBI Waiver services enrollment, providing comprehensive guidance throughout the process.

  • Provide step by step assistance throughout the Waiver program approval process with frequent contact.

  • Proactively follow up with prospective participants to ensure engagement with their enrollment and onboarding processes, maintaining regular communication with collaborating providers.

  • Offer and facilitate access to other forms of support throughout the NHTD/TBI Waiver program approval process including other NYS public benefits and community resources.

  • Meet all documentation timeliness requirements.

  • Collaborate with other internal departments regarding the clinical needs of the prospective participant

  • Participate in staff meetings and trainings.

  • Ensure compliance with all applicable local, State and Federal regulations and agency policies.

  • Participate in all mandatory ongoing training and professional development offered.

    Qualifications:

  • Bachelor's Degree in Social Work or higher, and two (2) years of experience providing case management/service coordination, information and referral services and brokering of community-based services for individuals with disabilities and/or Traumatic Brain Injury or seniors who are medically fragile.

  • Must have a general knowledge and understanding of developmental, physical and cognitive disabilities, traumatic brain injury, chronic medical conditions and/or mental illness.

  • Bilingual Spanish (fluent) is required.

  • Proficiency with Microsoft Suite and EMR systems.

  • Strong verbal and written communications skills, with the ability to handle sensitive patient interactions empathetically and professionally.

  • Excellent organizational and multitasking abilities, with attention to detail in communication and documentation.

  • Must effectively communicate both verbally and in writing.

  • Must possess excellent time management skills as well as the ability to work remotely and productively.
