RiseBoro Homecare Inc. Case Manager, Learn and Earn in BROOKLYN, New York
ESSENTIAL DUTIES AND RESPONSIBILITIES
Coordinate recruitment and registration process;
Coordinate participant orientation;
Responsible for completion of Individual Service Strategy form (ISS) for all participants; input all ISS data into the Participant Tracking System, including case notes;
Responsible for meeting with all registered participants on a monthly basis for counseling purposes; maintain counseling log;
Maintain participant portfolios; properly file participants progress/case notes and enter on a monthly basis in Participant Tracking System (PTS);
Refer participants to other programs or services if necessary and maintain supporting documents on file;
Ensure that participants graduate from High School or obtain HSE (along with Program Coordinator);
Attend DYCD required trainings;
Assist the Program Site Director in maintaining continuous communication with school administration for the collection of necessary documents to incessantly monitor the academic and behavioral progress of the participants, as well as to maintain updated documentation for participant files;
Assist with planning all Service Learning Projects as prescribed by DYCD;
Ensure that the participants have every possible resource to obtain program goals;
Perform related tasks as necessary at the discretion of the Program Site Director & Director Youth Employment Programs.
COMPETENCIES
Must have excellent interpersonal, communication and writing skills.
Must be detail-oriented, organized, and have the ability to multi-task.
Proficiency in MS Office Suite.
Bilingual in English and Spanish a plus.
EDUCATION EXPERIENCE
Bachelors degree; degree in Education and/or Psychology preferred
