Description

Comunilife Inc.is looking for an experiencedSocial Workerto joins itsBronx DHS Residential Program, West Farms. The Social Worker provides supportive services for clients at the West Farms Residence. Responsibilities include providing mental health related services to a caseload of clients with a history of street homelessness. The Social worker will be responsible for the provision of services and referrals with or on behalf of clients including, but not limited to, assessments, counseling, service planning, entitlements, and medical, educational, substance abuse, employment, mental health, and recreational services. Services and referrals should ensure the expeditious placement of clients into permanent housing and assist individuals in becoming self-sufficient. The social worker is responsible for providing clinical and case management services for his/her caseload as well as for maintaining case records for his/her caseload according to the requirements of Comunilife Inc., and all other regulatory agencies. Services and referrals should also include but are not limited to, training in the area of how to interview for apartments and/or supportive housing and how to complete housing forms.Comunlife offers all full time staff a generous benefits and paid time package.

Essential Duties:

  • Conducts mental health assessments and develops and implements comprehensive service plans for clients with mental health diagnosis.

  • Provides counseling and mental health referrals.

  • Responsible for the conduction of mental health support groups that follow the Wellness Self-Management Model.

  • Responsible for conducting initial intake interviews and assessments (as determined by site protocols) for all new clients and establishing case records.

  • Reviews the Comunilife and site specific policies and procedures with each new client.

  • Provides individuals and group clinical case management services for his/her caseload clients.

  • Meets with clients, at least, once a week in a face-to-face meeting to document the client’s progress.

  • Performs crisis intervention, assessment, advocacy, negotiation, referral, and case planning activities with clients as needed.

  • Educate clients in activities of daily living skills.

  • Conducts unit/bed inspections, at least once a month and reviews the results of the inspections with the client.

  • Escorts clients to appointments (medical, entitlements, housing, etc.).

  • Counsels clients and assists in problem-solving.

  • Provides information and referral services to clients as needed or as specified in the service plan.

  • Observes and assesses clients skill levels, notes progress in functional abilities.

  • Serves as liaison with various agencies, health care providers, mental health providers, entitlements, and employment and training providers in the interest of the clients.

  • Advocates for and assist clients in the negotiation of service delivery systems to ensure the maintenance of permanent housing.

  • Participates in weekly individual supervision with his/her supervisor, team meetings, and case conferences.

  • Completes incident reports and submit them to the supervisor for review.

  • Responsible for increasing knowledge and understanding related to the Multicultural Relational Approach and use in engagements with clients and staff.

Qualifications

Education and Experience Requirement(s):

  • Master’s Degree in Social Work or Mental Health Counseling

  • Two (2) years’ work experience

  • Experience with individuals living with HIV/AIDS, chronic mental illness, substance abuse individuals and their families a plus

  • Bilingual skills in English/Spanish a plus

  • Knowledge and hands-on experience in providing direct services related to the treatment and recovery of persons with HIV/AIDS, substance abuse, serious mental illness, and homelessness

  • Excellent oral and written communication skills; proficiency with MS Office and Google product suites
