JOB SUMMARY: To provide direct resident care under supervision of RN, while adhering to each individual resident’s plan of care and to ensure provision of care for all residents during shift while acting as charge nurse. Reports all cases of alleged violations involving mistreatment, neglect, or abuse, including injuries of unknown sources. QUALIFICATIONS: Education: Graduation from an accredited school of nursing and current licensure by the State Board of Nursing. Experience: Orientation training; Experience/ability to make independent decisions when circumstances warrant. Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care. Job Knowledge: Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to the health care field. Other: Must possess leadership and supervisory skills and ability and the willingness to work harmoniously with professional and non-professional personnel. Must be able to read, write, speak, and understand the English language. Responsible for actively demonstrating Putnam Ridge’s “PRIDE” philosophy of professionalism, responsibility, integrity, dedicated service, and excellence. ESSENTIAL FUNCTIONS: I. Charge Functions: A. Receives and gives report to other charge nurses and supervisor; Counts controlled medications with out-going and on-coming nurses according to procedure (in the absence of a second nurse). B. Assigns and directs direct care staff and other Licensed Practical Nurses when appropriate in resident care and nursing activities. C. Carries out resident/patient care plans with consultation with the RN as necessary D. Assists with resident care as the position demands. E. Makes rounds to all residents/patients at least twice a shift and PRN F. Investigates employee problems/incidents; deals with them according to procedure G. Notifies the RN of resident/patient problems (medical, physical, psychological, family, and physician). H. Makes rounds with the physician, calls physicians for changes in resident condition (after notifying RN) and transcribes physician orders. I. Handles resident incidents, emergencies, transfers, admissions and deaths, according to procedure and with the assistance of the supervisor. J. Participates in the care planning process. K. Completes all required documentation and paperwork, i.e. Nurse’s Notes, summaries, flow sheets, etc. L. Completes specific duties as required by shift assignment (i.e. night nurse checks each medical record for any outstanding orders). M. Meets with direct care staff at start and end of shift to facilitate report on resident status N. Evaluates performance for area of supervision. O. Orients new personnel and oversees participation and participates in related staff development and inservice training. P. Responsible for safety in area of supervision. Q. Demonstrates commitment to equal employment opportunity. R. Checks emergency equipment at beginning of shift. II. Medication Functions: A. Counts controlled medications with out-going and on-coming nurses according to procedure. B. Passes scheduled and PRN Medications for each resident; Evaluates the need and efficacy of PRN medications C. Follows all policies and procedures for administering and documenting medications. D. Checks orders for accuracy. E. Calls the pharmacy on drugs that need to be reordered. F. Calls the pharmacy on any discrepancies in medications and documents same and notifies RN. G. Assists with resident/patient care as needed. H. Keeps the medication room and medication cart clean and in proper order. I. Assists charge nurse with summaries, charting, other duties as directed. J. Directs and supervises direct care staff. K. Monitors meals in the Unit dining room and/or Main Dining Room. L. Assists with admissions, emergencies, transfers, admissions and deaths. M. Assists with transcribing physician orders. N. Makes rounds with physicians in the absence of another nurse or as assigned. O. Contributes to the care plan for residents/patients. P. Assists with resident/patient teaching as applicable. Q. Assists with resident/patient families and friends concerns or inquires. III. Treatment functions: a. Administers resident/patient treatments as per physician and/or nursing order. b. Evaluates the need of PRN treatments for each resident/patient. c. Follows all policies and procedures for administering and documenting treatments. d. Maintains treatment cart(s) in clean and appropriate order. e. Notifies the RN if any discrepancies in treatments. f. Reports to charge nurse on each unit upon arrival and departure if functioning on more than one unit. IV. Other duties as assigned PHYSICAL REQUIREMENTS: Must be able to lift, push, pull and move a minimum of 50 lbs. Must be able to bend, lift, transport equipment and residents. Minimum Salary: 31.00 Maximum Salary: 35.50 Salary Unit: Hourly
