JOB SUMMARY: Provide various resident care activities and related duties for the specific needs and comfort of the aged and infirm. Takes direction from RNs and LPNs to carry out plan of care. Complies with federal, state and local long-term care regulations. Certified Nurse Aides are able to identify competency in skills and techniques necessary to care for residents’ needs, as identified through resident assessment, and described in the plan of care. QUALIFICATIONS: Education: Nurse Aide Certification by the State of New York High School Diploma or GED preferred Experience: Experience/ability to deal tactfully with personnel, residents, family members, visitors, government agencies, and the general public. Prior health care experience preferred. Other: Must meet mandatory in-service hours by attending in-services provided by the facility. Must be able to read, write, and speak the English language. Responsible for actively demonstrating Putnam Ridge’s PRIDE of professionalism, responsibility, integrity, dedicated service, and excellence. ESSENTIAL FUNCTIONS: • Provides direct resident/patient care under the supervision of RN or LPN. • Follow individualized resident plan of care. • Performs duties such as: activities of daily living- bathing, dressing, assistance in nutrition and hydration, locomotion/ambulation, feeding, incontinence care, toileting, oral hygiene, skin care, grooming such as: comb/brush hair, nail care, shave. Bowel and bladder assistance, maintenance rehab, proper understanding of least restrictive device regulations, proper utilization of restraint devices, special care needs. • Provide for residents with special needs related to their diagnosis (i.e. behavioral issues, splints, etc.…) • Reports all cases of alleged violations involving mistreatment, neglect or abuse including injuries of unknown sources. • Documents resident care accurately and legibly as per facility policy. • Notes resident changes of condition and reports same to LPN/RN. • Answers call bells in a timely and polite manner. • Assists in provision of post-mortem care. • Performs vital signs as directed. • Performs tasks and responsibilities of assigned shift. • Assists in orientation of new personnel. • Reports on duty, listens to and/or obtains report from Charge Nurse, receives assignment. • Makes two hour and PRN rounds on all residents. • Keeps unit neat and clean; straightens, nourishment station, clean and dirty utility room and other areas as assigned. • Performs procedures outlined for Certified Nursing Assistants in the procedure manual. • Assists in the psycho-social aspect of the resident by cross training in recreational therapy. • Ensure the resident is out of bed, dressed and ready to participate in activities of choice. • Assist with transportation, if needed, to and from activities. • Provide assistance with ADLs during activities if necessary. • Assist resident to participate in individual activities of choice, (ie: setup of equipment, positioning assistance, providing sufficient lighting and space). • Inform residents of various activities available to them even when Activities staff are not available. • Other duties and responsibilities as assigned. PHYSICAL REQUIREMENTS: Must be able to lift, push, pull and move a minimum of 50 lbs. Must be able to bend, lift and transport equipment and residents. EXPECTATIONS: Treats residents/patients with respect, dignity, and compassion. Is confidential. Is cooperative and considerate to fellow employees. Is available and willing to assist as needed. Completes assignments as directed by RN/LPN. Is neat, clean and properly attired as per dress code. Is available for assigned overtime.

Minimum Salary: 20.50 Maximum Salary: 26.00 Salary Unit: Hourly
