Community Based Care of Brevard, Inc. Out of Home Care Supervisor (Brevard/Seminole) in Brevard/Seminole County, Florida

Salary: $57,000/yr Position Summary: The Out of Home Care (OOHC) Supervisor leads, guides, and directs a team of Placement/OOHC Specialists ensuring timely and efficient placement of children in the least restrictive licensed out of home care setting. T his position is responsible for the management and oversight of the placement team ensuring a safe and appropriate placement for all children and youth served. The OOHC Supervisor assists in retention efforts of family foster homes and group home providers, receives and follows up on any foster parent concerns and facilitates placement stabilization stuffing’s to prevent disruptions. This position also monitors sibling groups that are separated and maintains tracking for all OOHC children qualifying for APD services. The OOHC Supervisor is responsible for data collection and analysis, and f acilitates daily team hurdles to assess placement needs, assist in complex situations, and delegate tasks while ensuring compliance with contractual, regulatory, program, and accrediting body standards. The OOHC Supervisor ensures follow up and outreach for placements to support and maintain positive relationships within the Family Partnerships of Central Florida System of Care. This position participates in the 2nd tier On Call Rotation. Technical/Functional Expectations: Leadership-Essential Function: Problem solves, engages team, and assists with complex cases, multiple placements, significant behavioral and/or mental health needs, history of placement distribution, sibling groups etc. to ensure staff feel supported and that children are appropriately, safely placed. Collaboration and Communication - Essential Function: Strong Collaboration and communication with foster parents, group home providers, Case Management staff, Guardian Ad Items, and other stakeholders and provider networks to proactively engage and share information transparently to maintain open communication and support. Placement Tracking-- Essential Function : Ensure data entry is accurate and completed daily to reflect all current costs of care Oversight of Licensed OOHC Levels of Care: This position provides oversight of the children and youth placed in licensed out of home care including ensuring that Medicaid eligibility criteria is met and in obtaining prior authorization for Specialized Therapeutic Foster Care and Group Home Levels. Child Specific Outcome Measurements: This position provides oversight of child specific outcome measurement systems, collects, and analyzes data and report trends. Data collection: Develops and implements tools and processes to track critical data elements. FSFN Reconciliation: Ensures accurate and timely payment to foster parents and group homes through reconciliation to avoid or resolve payment issues. Over Capacity Waivers: Approves, tracks, and ensures over capacity waiver process in compliance with FL Administrative Code. Tracks over capacity waivers to ensure compliance and monitors to ensure appropriate follow up is conducted on homes under an OCW. Other duties as required: Job performance requires fulfilling other incidental or related duties as assigned, assisting, and training others, and performing duties of higher rated positions from time to time for developmental purposes. Business Experience: Child Welfare 3-5 years: This position requires a broad knowledge of Child Welfare policy, procedures, and best practices. Supervisory - 1 + years: This position supervises a team of OOHC Specialists, Valid Driver's License - Required to travel throughout the County and other locations for client and other meetings. Child Abuse Prevention or Child Welfare Experience: This is required to appropriately manage out of home care placements in the least restrictive culturally competent setting. Strength Based Family Centered Practice Orientation: This is essential to effectively support the System of Care and its values. Strong Interpersonal, Presentation, and Organization skills: This is required to effectively partner with various stakeholders and agency staff and handle complex issues in a dynamic environment. Utilization and Fiscal Management: This is required to effectively evaluate levels of care, be fiscally responsible and manage a continuum of services. Valid Driver's License - Essential: This is required to travel throughout the county and other locations. Educational and Experience Requirements: BA/BS - Bachelor’s Degree or equivalent required. MA/MS - Master’s Degree or equivalent preferred.
