The centers provide campus-based direct services and supports at 13 centers located throughout the state. This position works under the supervision of the Quality Assurance Department and is part of the Central Records team. This position completes documents and reports accurately and according to facility guidelines and time frames. Completes all filing of documents daily and weekly as required by the Active Record Order and Maintenance Guidelines. Keeps all records updated, purged with excess records filed according to the retention schedule. Maintains records in such a manner that they are accurate, up to date, complete and in serviceable condition. Provides information and instructions to professional disciplines that affect the operations of the individualand#146;s active record. Other duties include completing, mailing and tracking consents, and sending out Individual Support Plan notifications to families. Essential Job FunctionsAttends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.Files Annual ISP documentsFiles ducments into Paper RecordPrepare Pre ISP PacketsScan/Track and Upload any/all consents/acknowlegements in the electronic medical records that are mailed out and/or received by the ARC to/from LAR, LIDDA, and other disciplines as applicable.Update/make any new contact/address/name information changes on the Individual's contact sheet and email to the appropriate parties.Create iBook and Active Record Chart for new admissions, or when asked by QIDP if original is missing or damagedMonthly Medical Records scan callOther duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agencyand#146;s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.
